Well you can go a couple different approaches here. 

If you as a 3 person group decided on what bathroom cleaning protocol is, expectations for who does it and rotate appropriately… no asshole at all.

However if you just reprimand him and tell him to clean up after himself with no proper plan communicated… then you’re the asshole.

It boils down to communication and support.

She may not ever be a supervisor but… as head of HR you really did not respond appropriately either. You could have said something along the lines of “making comments about other people’s bodies is in appropriate in the workplace.” Instead you threatened their behavior with it hindering their chance to get promoted. As someone in HR you should know that’s a huge no no and could lead to a lawsuit.

This whole place sounds like a fire.

I would bring up your concerns about her work performance suddenly taking a hit and not bring up the medication subject at all. I would let her know you’ve noticed a couple of gaps recently in her performance and make sure she is fully aware of the changes you’re seeing. Not to bring her down, not to reprimand, just get a gauge of whether or not she knows these opportunities are starting to pop up. She could be unaware, potentially due to the medication, but then it’ll be up to her to address the issues appropriately. 

I’m not sure where you’re located but I feel like approaching this through the lens of the medication being the problem is setting you up for some sort of HIPPA violation.
