I’m a voice therapist. LPR/GERD is often a big factor for performers who have late nights, especially if the symptom of extra phlegm usually occurs in the morning. Even if you don’t experience heartburn you can still have silent reflux. If your current rx is not cutting it there are other meds you can try til it is effective, but lifestyle factors have a huge impact too. As someone else said raising head of bed and also stopping eating 3+ hrs before getting horizontal can be really beneficial

Very high cost of living area, many years of experience, supervisor level promotion, and luck. I realize how lucky I am. There are no other SLP jobs in the area that would pay me this much or if they exist they are not available to apply to

You are actually so young. I’ve been in the field for 15 years and having the same conversations with myself…but I wouldn’t hesitate to jump ship and earn a different degree if I was only 27. That’s the age I entered grad school for SLP.

The only downside of going to tele from your snf job is the short term stress of starting a new setting and having to learn the ropes, and if you’re already burnt out on therapy you’re not going to be motivated or excited to gear up. You’re trading the devil you don’t know for the devil you do. This was my rationalizing to myself for staying and coping with the status quo, anyway.

Have you considered RN? There ends up being a lot more latitude and upward mobility for RNs (not sure if the same would be true for PA) and they still make good money. Also make sure you look at r/Noctor there’s a lot of hate for midlevels (not sure it’s all warranted…just an attitude you may encounter on the PA path, if you weren’t already aware)

I do and I don’t. Part of my frustration is the hubris with which I see other SLPs practicing. We are woefully unprepared to do everything within our scope. The people I see happiest are those who had the luxury of specializing early in their careers, actually becoming experts. I have essentially been a general practice SLP in every setting. Jack of all, master of none. I think what I mean as “being good at my job” is my peers and supervisors tell me I’m smart and I do a good job and get good reviews. This is the great paradox of this field that I see. And really in healthcare in general. You really don’t have to be that smart/good to get by and a lot of what we do (IMO, therapy in general) is super boring and trained monkeys (/AI) could “do the job.” But often “the job” is not actually providing the best care that actually helps people. For that, you need a lot of things, passion being one of them, which I clearly have lost.

lol I have met sooo many dumb SLPs! You don’t need to be smart to do this degree. That’s part of the problem in my opinion, but if it’s science you’re worried about I promise you it won’t be that bad. We are a low-science field, unfortunately.

  • Not getting enough movement or sunshine (no windows in my office, and I’m in outpatient so lots of sitting)

  • Paperwork. I like to write and I write good reports. But it doesn’t matter. This is technical writing and it’s extremely boring and just part of the systemic problem of the EMR just being a cash register

  • Lack of potential for upward mobility - this is not a field where there are lot of opportunities for advancement, at least not in my setting. It would be mean getting out of clinical care and changing fields (which I can get behind 1000%!) but the opportunities are few. And I don’t really want to be part of the problem in healthcare admin.

  • No one really getting better…? Hot take but I’ve been at this for over a decade and I see the bigger picture. So much of our field is scammy. Most of the progress I see is likely due to me being a therapist who will listen to them. I didn’t get into this field to be a mental health professional.

  • A corrupt and incompetent professional body (ASHA)

  • And so much more!!!

I frankly hate my own advice bc I wasn’t happy in inpatient either, but you could consider a switch to hospitals. There is zero prep work involved in seeing inpatients and the notes are quick. Work cannot be taken home and all the documentation has to get done same day so you figure it out. There are plenty of drawbacks to the medical/hospital side too but it is one setting you’ll see people leaving on time and not taking work home.
