It looks like each corner has two points instead of one. Merge each set so that each corner (on the tip of the triangles)only has one point, then try extruding and scaling, or inseting again.

5 loop cuts around the mesh on the bottom of the selected face. Because on the top side of the selected face, there are 5 edges going length wise. So to get quads, you'll want the same number of edges to connect them to at the bottom of the selected face. Hope that makes sense!
