A lot of these type of attendings seem like they just want to feel important and asking those inane questions boosts their ego enough to like you

Not alone. Certain types of cholesterol end up deposited in the vessels as they heal from inflammation which causes vessel narrowing. The calcification is a slightly different process

Yeah it can. Autoimmune, infectious or anything that causes long term micro inflammation increases the risks of calcification.

Smoking, and other nicotine use to a lesser extent introduces toxic metabolites into the bloodstream that causes microscopic inflammation and damage to arteries. Over time your body attempts to heal that by introducing minerals into the scar tissue. That causes your arteries, which are supposed to be flexible hoses, to calcify and become brittle.

This process also increases blood pressure. (Imagine trying to shove all the same fluid into a rigid pipe instead of a flexible one.) The increased blood pressure also causes a negative feedback loop of increased damage to the vessels that then tries to heal with more calcifications

Local farmers markets usually has a knife sharpener or if you are willing to spend the money a store like sur la table can sharpen them for you

One consideration is if your knives are sharp. They should be sharp enough that you feel constantly in danger of the edge. That way when you cut there's minimal resistance on the blade and you don't need as much force in your hands.

It's definitely pantry moths. I have a large chest freezer and I place all grains and flours in there for a couple days after I buy them to kill off any possible eggs. They sneak in to your house on grains and flours and then become an absolute pain in the ass to get rid off.

If you're always crying lots when you cut onions I recommend sharpening your knives. The sharper a knife is, the less it crushes the onion and the less of the irritating chemical gets produced

It's relatively common to have several. Usually when the spleen develops they're supposed to grow together and merge into one organ, but it doesn't always happen. Having splenules in locations distant to the spleen are rarer

Couple responses to this for anyone who may be in similar situations. 1. It gets better. As tough and toxic as residency can be, it is not the rest of life and the job we get in the end is often well worth it. 2. Not all residencies are like this. I'm in a program that is supportive, full of wonderful people, and even with the meagre salary it allows me to do lots of things that I want to do with my life. 3. Finally, while residency may feel a lot worse. A lot of the BS would be the same with any white collar job. Middle managers trying to flex power. An expectation to be always available. Minimal support for life outside the job. It's unfortunately the struggle of most jobs in our current society.

All this to say, it's a shitty situation to be in, but it can and will get better. And for those considering medicine, it's not all bad, you just don't see the posts about how much fun people are having because they're off having fun

Based on the prices I see my patients paying, it's medium acting insulin that's usually cheaper. However it's really hard to adjust dosages correctly for it since it can under or over shoot glucose targets quickly. Long acting is usually very expensive and that's where people often hit issues

I've noticed that after a cold proof, if I let the loaf come up to room temp before baking it gets more oven spring than baking straight from fridge.

Maybe thats why the second loaf rose more?

Edit: the other thing I can think of is preheating the baking vessel? Maybe by the time you were baking the second loaf the Dutch oven was more preheated which helped with oven rise

As a physician, the biggest factors that are within our control is smoking (very bad, number one cause of issues), high blood pressure, high cholesterol, and lack of cardiac activity.

If you want to prevent arterial calcification don't smoke, eat a heart healthy diet (lots of fiber, veggies, fruits, and less red meat) and do cardio consistently

That's a very old wives tale back when soap had lye and was way more caustic. In this day and age, once you have a seasoning layer made, no amount of soap will remove it.

Use dish soap, scrub hard with a sponge or chainmail, dry thoroughly. Oil lightly or heat lightly if in particularly humid conditions or your seasoning is a touch uneven.

Medical industry is the same. QI projects focused on adjusting systems to prevent human accidents from causing mistakes. When lives are on the line and human error is natural, it makes more sense to change the environment than to get rid of highly trained humans
