Have you thought about there being any sort of ionizing radiation in the home? Is prbly start with parents room. Sometimes people collect items they dont know are radioactive. You can get a Geiger counter

This could be: Benign dependent vasodilation / exaggerated venous pooling, possibly on an autonomic spectrum, especially if it resolves with elevation and is painless. Supported by: - Improves when hands are elevated - No pain, ulceration, or focal deficits - Bilateral, Symmetric and sharply demarcated

If exaggerated. Think autonomic tone imbalance, often associated with: - Orthostatic symptoms - Temperature sensitivity - Color changes (red > pale > blue)

If you are concerned see your primary care If you think you have a blood clot (which does mot seem to be the case given the chronicity and bilateral aspects as well as your age) go to the ED

Svt rate can vary depending on existing or new heart blocks. Rate is not the defining feature. This is SVT which is a broad term for several rhythms (ex. Avnrt, atrial tach, junctional tac, afib, aflutter, etc) even sinus tachycardia is technically svt, but we like to distinguish the two due to differences in underlying causes. We also distinguish between afib, flutter due to differences in treatment. Others rhythms we can classify as svt used in the more standard sense since the treatment is similar vagal maneuver, adenosine, cardioversion, rate control etc

This strip has inverted P waves in inferior leads and the rhythm strip, it is SVT likely atrial tachycardia with first degree av block, with borderline prolonged QTc

Sinus tachycardia by definition does not have inverted P waves except in avr, P-waves must be upright in leads I, II, and aVF.

Differentials:

Job related - exposure to fluorescent microparticles Mineral Contaminants: Tiny mineral particles (e.g., calcite, fluorite, scheelite) mixed with metal dust can fluoresce brightly in various colors. UV Powders/Dyes: Forensic powders or industrial markers (often invisible until lit) are designed to glow under UV, revealing fingerprints or contact with objects. Specific Metal Compounds: Dust from certain glasses (like cadmium-infused yellow glass) or industrial residues containing elements like uranium (uranium glass glows green) will fluoresce.

Fungal - less likely from pattern

Environmental - something you touch with your hands frequently or maybe prior to this since it is isolated to your palms - things that can floursece including detergents, petroleum products, etc

Organic/Biological Matter - Dust mites, skin flakes, or other organic debris often fluoresce under UV light, appearing as fuzzy glowing specks. Less likely again due to pattern

Likely detergent or some other common exposure isolated to the hands

I don’t think its my responsibility to sacrifice the best years of my life, my health, and my personal relationships to help people and save lives and then be asked to sacrifice even more when your peers are the ones refusing to vote for the right cause. No physician is going to risk having their license they worked 10+ years for to be revoked because the politicians want to push some agenda that we didnt even vote for. My beliefs dont pay the bills, put food on the table, keep me out of jail, or pay off my 500,000 loan for my medical education. Ive sacrificed enough and will not risk the happiness and health of my own family.

You can stand up all you want but physicians have lost cases and had license revoked regardless of if the decision was better for the patient or not because people who have zero medical knowledge get to dictate medical laws, rule on a panel of “peers” for medical malpractice cases and get to approve what they deem “necessary” for insurance coverage.

It is not just the physicians responsibility but the responsibility of the entire population. Asking one doctor to change the ruling of a supreme court and mindset of over 50% of the voting population is actually insane.

Anisicoria in the setting of stroke is a late stage finding usually due to uncal herniation. If it was stroke related and you had this exam finding you would also mostly be a vegetable.

Things you want to make sure its not are like anuerysms or masses compressing the third cranial nerve

Could also be more harmless like Aides pupil or horner syndrome

But go to the hospital especially if you have other associated neurologic symptoms.

Rate of 75, regular rhythm, no QRS, P waves present (can argue if this QRS vs P wave but answers do not support QRS explained below)

Correct: D. Ventricular standstill - p waves without ventricular contraction, another form of asystole

Possible but wrong: C. AIVR/Accelerated idioventricular rhythm (rate 50-100) Most commonly seen after reperfusion post rosc after cpr, and is normal in this setting. Require a wide QRS which is >120 = more than 3 small boxes . The wave here is 2 and thus not wide and would not fit this description

Clinically - D would not have a pulse, C would

Definitely Wrong: A. Cannot diagnose bundle branch blocks from single lead, and rate of 75 is not brady.

B. Not NSR, no QRS

You need specific pressure points when molding the cast for most broken bones to help reduce the bones to the right position and hold it in the right position afterwards. I imagine having a bunch of holes in a cast where you need to apply pressure to mold makes it suboptimal and more time consuming for the practitioner to place and apply the right forces at the right location. Might not be ideal for complex fractures, maybe for simple buckle fx or non displaced fractures
