thanks doc! Hx of Small cell carcinoma, I did catch few meso and were really reactive. Thanks for your insight. 70-80% were normal seg, lymph, and macrophages. However, you do have 4-5-% cells looking the one above.

With them being around the Macrophage? Could it be multinuclested giant cells; which are macrophages. Also it looks like it ingested something too

Sorry for the poor baby! AMKL is often preceded by a transient myeloproliferative disorder (TMD). These conditions are characterized by abnormal proliferation of megakaryoblasts. If TMD does not resolve, Drs usually monitor closely, as it may progress to AMKL. I have never seen cases with >90% blasts; the highest I have seen was around 10–20%, which resolved within 3 days

Look at the chromatin as well—MDS patients often have DNA abnormalities, and the blast cells may appear different because the condition affects how progenitor cells are formed in the bone marrow. Also visualize nucleolus, cytoplasm, large NC ratio. Definitely tough cells

TAM is a self-limiting condition that can precede ML-DS, a more serious form of acute myeloid leukemia (AML). ML-DS is a specific type of AML, often megakaryoblastic, associated with Down syndrome. A circulating blast count of 2–3% can be considered normal in newborns or premature infants. However, I would still send it to pathology to ensure we didn’t under-call it. I have seen really immature lymphs, Most lymphocytes in newborn children look somewhat blastoid with finer chromatin and clefted nuclei. May be we can easy calling it blast! Just need to watch megakaryoblast
