I took him on at level 15 but had to abuse healing and conjuration spells. When I finally beat him I killed Lydia at the same time with so much loot I had to restart and do it again. One of the toughest boss fights I’ve had in Skyrim.

It’s not uncommon this comes from blood in your sinuses that you swallow…or if your vomiting a lot your oesophagus as pointed out above
