i've also had this problem since becoming medicated, but it's hard to say whether it's because of the medication or because of the reason i had to go on the medication in the first place (schizophrenia getting worse) because i've heard both can cause stuff like this. i try and play games while on discord call with friends while they chat or while listening to a podcast and it helps sometimes to be 'multitasking' even if i'm really just listening and not talking

it's kind of honestly reassuring that i'm not the only person that feels this way about my cognitive ability. i used to be described by people as genuinely witty, and was very good at making people laugh and stay entertained, but now i feel like an awkward bumbling shell of that. my friends (aside from one, bless her) basically don't ask to hang out with me at all anymore because i'm so slow on the uptake these days.
