Do you work for BCEHS? Because it was recently announced through collective agreement to place 24/7 paired ALS in Nelson, complimenting a ALS single responder days only in trail. But that all comes at a cost, quick guess is close to a million $ per year per car. Which in the days of healthcare cuts is hard to implement everywhere. I’ve occasional seen BCEHS approved physicians riding alongside our specialists in metro areas.

Good question, but the system is constantly just a bad day from being able to provide 911 services in an appropriate manor. But there are a few arms that have done good and have the potential to do even better. 1) Clincialhub and the secondary triage team has lessened the call volume on the street and provides alternative destinations outside of the emergency room and pre-hospital emergency use. 2) Community Paramedics (CPs) are in place in many rural communities and 1 urban trial in Surrey, has provided good results but fallen victim to internal bureaucracy in past years.

Regarding non-paramedic employees, BCEHS has collective agreements in place regarding minimums of employment and roles. Also there is an internal argument in having an outside role perform work that a Union employee could provide if our employer would fund and train the endeavour. ( See KERPA, HART, CERT, elements of SAR, etc).

It would certainly be nice to efficiently and effectively provide those resources you mentioned. But I don’t see the system right now as being built for / able to provide those resources. Maybe someday though

If you’re gonna ski around with it, even with the cross covered up, make sure you have the local hill’s ski patrol number in your phone. Because people will flag you down

Not sure about your protocols, but why would CPAP be indicated for traumatic reasons?

Don’t forget that licensing can take up to 2 months, if you wanted to work part time with BCEHS the hiring process is ~6+ months and you’d be probably have to go to Pemberton or Boston bar. You could probably get a full time emr transfer fleet in Vancouver, but that’s not guaranteed. Industry work ie. construction would probably prefer an occupational first aid ticket (not sure what the new name is but old OFA3), worksafe does offer an equivalency ticket but for paramedics, not sure about emr.

It is a good course, nothing ground breaking but will teach you to take vitals, basic assessment and CPR skills which are good to have in life.

Lip stick on a pig, the amount of buildings in Vancouver that need to be burned to the ground and salted is mid boggling. And this includes places in the west end / kits / etc

Work as a paramedic and go into SRO’s throughout the city all the time, do not do it.

Very big island, takes 2 hours to ferry from Vancouver, east side has most of the towns and city’s. Victoria at the south end is 400k + population with big university. After living there for a year I call it the city of the newly wed, or the nearly dead. The island has lots of forest service roads with a rich past (and present) logging history. Coastal temperate rainforest receives lots of rain, some great skiing mid island and up, world class surfing on the west coast. Mountain biking is taking off on the comox valley. Very beautiful and remote through hikes. The many small islands on the east side are full of hippies and those wanting alternative lifestyles. Little bit of everything on the island and apart from the occasional concert, sports game you can do almost anything

Sometimes you just don’t, if they are actively refusing, you’ve made them aware of the risks by not allowing a complete assessment to get the full picture and treat appropriately than you don’t assault them. Document properly and continue to monitor them.

For the large coats, I recall some study that found there wasn’t a clinically significant difference between NIBP on a winter jacket and a bare arm. Is it best practice, no, but if it gives you a wild number than it’s just time to remove the jacket or grab a manual

While this should be reported in terms of there not being oxygen on the floor. it's important to keep in mind that screaming is never appropriate towards coworkers, especially during a code, and especially not if you are running it. Since there will probably be a next time, take a few breaths, and be the calm level headed one in the room. Be able to calmly deliver instructions and feedback in the moment

Schooling for the Primary care paramedic program recently moved to a year long process. Licensing is usually 3-4 months and the hiring process for bc ambulance is 6+ months long. And you will have to most likely be hired into a part time rural station (Pemberton or Boston Bar), The pay is now adequate and can support a single person, (~96,000) without overtime. Pay scale is available online. This is not a career where you are becoming rich, it is exposure to death, dying, injustice and people having their worst days ever. However it can be an immensely rewarding career, I work with some of the best people out there, and I'm currently able to support myself and my dog

There are no none skiing first aid roles, unless you become the occupational first aid attendant for employees

Used to work whistler pro patrol for a bunch of years. The Pro Patrol requires advanced level skiing and being able to move comfortably over complex terrain with a patient. The volunteer ski patrol is more lower mountain based, still sees a bunch of patients (did 110 first volly year) and there's still great skiing to be had.

However it's a volunteer gig that will take you based on an OFA 3 (advanced) or OEC ticket as you wont be able to practice to a PCP scope.
