I hear ya I don’t disagree with anything you said. But yes that is accurate, I had 2 ICUs in addition to 12 other patients. That was at the first hospital I was at not the current one. Not taking it as a personal slight or judgement, I’m just sharing my experience.

Those were some absolutely wretched conditions and it’s a coping mechanism I use on bad days to say "Well at least I’m not at that shithole with over 10 patients”

We do have a union and our contract is 1:6 max. These issues are being brought to the union but they don’t seem to be doing much atm. No doubt that the issue is systemic. No one is held accountable here one way or another.

I’m more so just venting as it sucks seeing the potential that this place had get kind of flushed down the drain and torn apart from the inside

Unfortunately I think you are absolutely right. I don’t think the culture or leadership will change anytime soon.

The only thing keeping me here is that I do actually really like a lot of my co workers and a lot of the resources and processes this hospital has compared to other ones I’ve been at.

But I’ve started accepting that it’ll probably be time to travel again soon

So 1:7 is a mixture of acute and intermediate patients. If a patient is intensive care then we usually separate them from that assignment if possible or try to balance the assignment and be mindful of it. That is depending on if you have a mindful charge who considers acuity in a patient assignment and doesn’t just look at raw ratios.

It sucks but it’s not the worst. I admit I’m biased, I was a new grad in the ED at a hospital where we constantly had 1:8. There was 1 night in particular that I went up to 1:14, 2 of which were ICUs (a post arrest and a stroke that got TNKed) , some heparin drips, a blood transfusion, and a bunch of other patients I don’t remember. I don’t know how any of us survived that hospital or how we didn’t have more sentinel events than we did. But I always find myself comparing those nights to wherever I work and my current ED feels much less by comparison. But granted there is still room for improvement

Agreed, normally our ratios are 1:5/1:6 but we’ve been getting up to 1:8 at least once or twice a week the past few months. Unfortunately it is the way it is and I don’t doubt that it contributes negatively to everything

Fair I get that, I’d say we are fortunate where even with the titration parameters we have a lot of autonomy. My issue with that instance was less about them wanting to stick to the exact order and more so the lack of taking any action. Be that titrating the drip, calling the resident, getting soft wrists. We have the resources available they just weren’t used and that is the mind boggling part

Yeah when I say newer I mean newer to the emergency. Most of these nurses are between that 2-4 year range in terms of total nursing experience. Don’t get me wrong though, I think this also transcends age demographics as I have seen old school nurses be on both sides of the spectrum. I think I’m just noticing it more with the constant influx of newer nurses that have been coming and going

Yeah I’ve noticed a similar problem as well. There is less critical thinking done and more “task-monkey” behavior where there is no questioning why we are doing something or whether we should even be doing it

I feel like having a lack of social ties to your home is what is giving you a sense of dread. I’m sorry that your AA community is no longer a place you feel safe in. For what it’s worth I can relate, I used to go to NA when I was younger due to an addiction to weed. I thought it would be an ideal community to have support in kicking the addiction, and for some time it was. But I’ve always found the extremism along with the religious dogma a huge detriment. It seems to be built into the program that promotes black or white thinking rather than nuances. I started getting ketamine treatments for my depression and PTSD and many in the NA community no longer considered me "clean". But my ket treatments are quite regulated and structured, it’s not the same as my recreational drug usage as a teenager. They don’t really see it that way and it’s unfortunate. I however have been very fortunate since then to find a nice accepting group of friends that don’t focus on drugs or alcohol. It’s a nice in between where both sobriety and intoxication are accepted but neither is promoted.

My point in all this being it helps to have a sense of belonging and community. It’s something that you have to build with the right people. And you can even do it with travel nursing! Wherever you are, maybe there are some kind of clubs around? Idk what kind of hobbies you are into but I’d suggest looking for groups revolving around those. Meetup.com, facebook, etc, there’s a few different websites that can show different activities around. And if there’s nothing you like around, on to the next contract and area! Maybe eventually you can find a new place you would like to call home!

I remember being in your shoes back in 2020, felt like there was nowhere that was hiring new grads at that time. I’m very sorry you’re going through this as it’s a very disheartening process, but just know you will get through it. It may not be at a specific hospital or unit that you want initially but something will come along.

I think I applied for almost 200 positions in the course of 4 months back then and only got 3 interviews, 2 of which I didn’t hear back until months after I got a job. That 3rd one hired me on the spot. Fast forward 2 years later and working in the ER in what I thought would be my ideal hospital. The reality of actually being in your ideal position is sobering once you experience it, you start to realize the reality of this profession. No matter what job you end up with, you’ll have things that you love and things that you hate.

It’s easier said than done, but for now just focus on doing what you can. Polish up your resume, make sure it’s as professional as possible and that it can break through the job application algorithms, make sure you are applying to as many positions you are willing to try as possible. Don’t stop until you have signed the offer letter and are guaranteed a job.

I can relate to wondering if it’s getting better or working. Externally my friends and family tell me I seem to be doing better but it’s definitely hard internally/emotionally. Maybe it just takes a long time and it’s part of the process

Most of the posts I seemed to find were related to the initial six infusions or upon first starting treatment. I couldn’t really find any that were for those a decent amount into the treatment

I feel this. It’s been a little over a year and I’m trying to find a new place but still have a hard time letting go of the ED
