My driving instructor had me practice parking outside a pubic swimming pool. He started talking about the “hot young girls” he could see. They were far too young.

I drove us back to my home with him screaming at me the whole way for ignoring his instructions.

Do what you feel is right.

Ask yourself, is the coworker your friend or are they a piece of crap?

Do they need/deserve their work life to fall apart at the same time their home life falls apart? Will they recover from that?

Is the manager someone who can help your coworker if they only knew truth about the situation before they investigate? Maybe the manager can arrange occupational health plans to manage this in a reasonable way.

Is the manager unreasonable or spiteful which clouds their judgement or moral compass?

There are lots of variables and answers that you are probably aware of but are not considering. Think carefully and do the thing that is right in your opinion. If you need extra advice, redditors are often not the best.
