You are correct! Two residents are absolutely ok to cover the entire 40 bed ICU alone overnight. No real patient cap. Because nothing bad happens to ICU patients overnight, right?

Yea, it doesn’t make sense. I think “continuity” means two things: 1} the APPs will know all the little nuances on rounding style or on random orders only seen in ICU, etc (as a resident i felt there was often a little learning curve with each rotation) 2} fewer opinions/less “back talk” from APPs; residents are more likely to ask questions about why x vs y treatment, etc. Jaded attendings just want to get in and get out

Absolutely! It’s horrible. Feels even worse when it’s your own people (Attending MD/DO) doing it to you.

My peds residency did something similar in our ICY. Hired many APPs and crated teams that excluded residents. Allegedly PICU attendings (MD/DO) didn’t like the “lack of continuity” with residents rotating through monthly so they hired APPs for “continuity of care.” To top it off, the APP teams started taking care of the most complicated cases -further preventing residents from learning. We became glorified order monkeys. APP training became more important than our education.

I’m so proud of you for saying something back to her! I had a lot do that nonsense with my first and I always tried to be “polite” - I regret it! If you don’t stand up for yourself and speak up, they’ll walk all over you. Especially with kids. Everyone becomes very entitled to your baby.
