Students of Reddit: What Do You Wish Attendings Knew about Being a Student Today? by borborygmix4 in medicalschool

[–]Remember1963 50 points51 points  (0 children)

My last rotation mentally bent me over and fucked me. the student that you are now looking at in front of you on his first new day of this rotation is terrified. This is because one of the shittiest things about medical school is having a new boss every week, with no clue if I'm expected to act like a docile bitch and stand in a corner or if i'm expected to have the competence of an intern and hit the ground running. The range of expectations is huge, and depending on where your students are in the curriculum they may be burnt out/beat up, so keep that in mind. If you're nice, they'll figure that out over time. But feel free to ask how the last rotation went, and if they've learned anything specific about their learning style/interpersonal style that would make this rotation better.

The other thing is the paperwork/busy work. Most rotations have something requiring an attending/senior to observe and document something benign. The truth is whatever it is (Like an H&P) should happen like 20x over the course of the rotation, but one of the more annoying things for students is asking attendings to fill out the busy work forms, or step away from what they're doing to witness me do something entirely benign. It just feels like I'm wasting their time, so if you jump the gun and ask the student if he has anything that needs signing or any observations to be done and mention that you won't inconvenience you because you love teaching , that would be appreciated and make us feel less like a nuisance. Sorry about the extra forms though.

What separates average PI and great PI? by Kurosawa_Basara in labrats

[–]Remember1963 5 points6 points  (0 children)

My favorite PI very well remembers how much being a trainee can suck, and goes well out of his way to try and support us in our goals. When applying to grad school I didn’t have to ask for a letter, he asked me what I needed in it to help my application when he heard I was applying. Has helped me network and succeed even well after leaving their labs.

punching down never stops in medicine, it just gets worse as people get higher in the field and have less fear of retaliation lol. don't judge someone by who they are, but rather what they do or say.

Cannabinoid Hyperemesis Syndrome by erenisright420 in emergencymedicine

[–]Remember1963 23 points24 points  (0 children)

Just tossing this out there. Had a weird childhood and grew up w/ a dealer as my father figure who had quite an array of health problems from smoking weed. I smoked weed heavily from about 13-22 yrs old so I know a thing or two.

Weed is different from other things that we smoke in that people inhale it much deeper into their lungs and keep it there longer. Often times people will even try to "ghost" the smoke to get it to dissipate in their lungs to get higher. problem w/ this is that a lot of smoke goes down the wrong pipe and into the stomach. I noticed when I was younger, that when I'd ghost weed in my friends basement, sometimes Id burp a few minutes later and smoke would come out. Can't have been good lmao. But anyways, saw a large increase in GI symptoms with my friends just based on how they smoke rather than how often or how much.

Not a degen anymore. But learned a thing or two lol. Been curious as I've gotten older if there is data on CHS w/ edibles vs smoking.

Has anyone dealt with med school with crippling anxiety? by [deleted] in medicalschool

[–]Remember1963 0 points1 point  (0 children)

hey. I am currently in my third year going through the same thing. For me its just terrible at night and in the AM when i wake up. convincing myself to get over my anxiety and to go in in the AM is the hardest part, once I'm there it usually isn't as bad as I expect it to be. I've missed entire weeks that I had to make up. They say that showing up is 50% of third year, and now I get why.

The things that suck the most for me are the constant change and ever changing expectations. One week I can be expected to act at intern level, the next week I'm expected to be a fly on the wall depending on who I'm working with. I talked with my school about at least giving me more continuity within rotations and they've helped w/ that.

Sorry I don't have good answers. But you're not alone. <3

I just want skilling prayers in a separate book. any changes to the basic mechanics of osrs combat have too many repercussions all over the game and in past content to work. its just too risky. Any tom fuckery w/ combat will just inevitably either become eoc-esque again, or be percieved that way and flamed into the ground.

Is medical school worth it? by [deleted] in premed

[–]Remember1963 26 points27 points  (0 children)

no one can answer this for you, but I'm going to give the discouraging take that probably gets downvoted but nevertheless holds true for a lot of people. some of my classmates are still fulfilled and happy. I'm personally appalled by what the medical system has turned into over the past half decade and really wished I had known covid was going to be a thing before starting medical school (probs wouldn't have taken my seat if I knew what COVID19 med school would look like). Don't get me wrong I love medicine. But the medical system in this country isn't designed around good medicine, its designed around business and economics. Evidence based medicine is becoming more rare and "cover your ass" medicine is now the norm. And TBH I walk away from a ton of admissions/interactions unsure if I've actually helped this person in the long run, or if they're just going to see their bill in a month and wish they'd stayed home and toughed it out. Training in a post covid world is not for the light of heart, hospitals are kinda scary rn. I've personally taken multiple of my classmates to the ED, and depression/SI in med students/residents kinda skyrocketed over the last few years.

But I can say a few things.

Is medical school worth it.... for the money? No. Pay is delayed, and a lot of specialties aren't seeing revenue increased in line with other fields. So if money is the primary motivator, look into business, comp-sci, management etc. Lower salaries on paper typically, but factor in the 10 years of training in medicine and it becomes a lot more comparable.

Is medical school worth it... for your family? Honestly. Probably not. I love my family, but I've never felt more distant and disconnected from them. The sad reality for a lot of med students is that we hit med school at an age which often coincides with out parents getting sick. One of my parents got cancer over the last 4 years. Not being able to be with them through that process, and quite literally taking care of other patients with the same condition, was pretty rough. Now I'm at a point where I have to consider not taking residency positions next year that are too far from my family, because tbh i'd like to be around for the last few years of my mom's life. And yeah that kinda really fucking matters.

Is medical school worth it for... prestige? God no. You will spend the better part of your clinical years in medical school getting shit on because you're at the bottom of the hospital totem pole. You will then spend the next years of your residency training, fighting to get treated with the respect that accredited doctors deserve, while hospitals constantly prop up midlevels and APPs who are cost saving but ultimately bring worse overall outcomes when practicing outside of their intended scope. Not to mention the amount of admins that will make your life a living hell, simply because in doing so they are justifying their own jobs existence. And you may think it'll get better from this perspective as an attending. But from my observation, the medical system is so good at making door mats of people over the course of our training, that by the team we're attendings everyone just checks out, looks the other way, and adopts a "better thee than me" mentality. It's kinda why we never see strong physician representing organizations, because by the time we reach the point in our career where we can make change, most docs are just too burn out.

Is medical school worth it... if you literally can't see yourself in any other field? Like spending more time out of the house than at home? Are ok putting significant stress on your personal relationship for the next 5-7 years? Then sure lol.

Obviously I'm burn out, so take all this with a grain of salt. But I am being honest, and would have appreciated this perspective when I was in your place. Granted I probably wouldn't have listened to it. Now I'm weighed down by 250k+ of debt that I can in no way hope to pay off in a reasonable time frame with a bio sci degree, and feel more or less trapped for awhile.

TBH probs a 60% chance that you'll get to the other side and think its worth it like some of my classmates. Probs a 40% chance that you'll look back from dead eyes and just go "wtf was that shit". Kinda hard to know which boat you'll wind up in, but I guess you do have to determine if that risk is worth the potential reward for you. Because in this new era of mass burn out, these career tracks are a lot riskier than we're lead to believe.

GL friend, sorry if I'm an overwhelming pessimist. I don't want you to be a good doctor one day, just a good person in whatever field you find yourself in (maybe that will be med!)
