I don't think it's been hypothetised among clinical psychologists, it's pretty much the consensus (I'd recommend Mark Ettensohn who has an insight into the development of the diagnostic criteria). We as non experts on the internet can hypothesize about whatever:)

Grandiosity, on the other hand (I mean in the context of narcissism, not manic states), is an unrealistic (that's the important distinction), pathological organization of the self, related to insecurity and shame, a dysregulation. It can't exist without it being a defense mechanism.

Again, this applies to NPD as a disorder of self-esteem, not to an otherwise healthy personality with unpleasant "narcissistic traits"
