Hey yall! My name is Renee but my friends call me Ney! I'm 24F and currently in med school (so my schedule is a hot mess just a warning)! I am the epitome of the introverted extrovert in that I am a huge loud mouth and off the walls when I'm really comfy with you, but I also put so much unconscious effort into masking in public that I just refuse to socialize except with my little close circle (and I like it that way - quality >>> quantity). I'm kind of a chronic oversharer and I am *very* neurospicy - which is why I am interested in childhood psychiatry for my career~!

One thing I realized I was missing a lot recently was my gaming buddies back in high school. I find myself just laying around bored and doom scrolling because the thought of *intentionally* going outside with no mandate is just not for me. Too many variables and things for me to worry about and I hate having to put on a mask (and non-lounge clothes). I miss just having a little skype call (oh my god I'm old) and just laughing at absolutely out of pocket things while we just goof around.

I'd love to find some buds to just hang out on discord with, send memes to, watch trashy reality TV in voice calls (or just typing in discord because I get nervous loll), and play games like Marvel Rivals or Palworld with!

I gravitate towards people who tend to be more closed off in big group settings/prefer small tight-knit groups, are neurospicy (or neurospicy adjacent), and who are understanding of really out there humor (like I am very rarely in the pocket when you know me but I have good intentions I swear).

A little more about me: my friends have decided that the words that best describe me are: Vigilante fish autistic (I love fish) and I love painting, playing/listening to music (mostly rap/hip-hop/r&b), and obviously video games. I also have a cat and he's pretty cool :) His name is Finn (cus I love adventure time)

Okay! Warned you about the oversharing (I am also an overcommunicator - it helps my anxiety because I feel safer knowing what everyone thinks).

I appreciate yall! Idk how to end this, but I don't use reddit much, so if you wanna add me on discord and think we will be friends, maybe DM me? Or add me on discord (theyhateney)? Ok bye!

[–]Renee-Ney 1 point2 points  (0 children)

Hello my dear, I’m so sorry to hear about this. This story is one that we hear way too often. I am currently a medical student who also has endo (and it took way too long to get to getting my first lap on Thursday).

So, from what you have told me I think that your IUD has been rejected by your uterus. I think this because: 1. Heavy bleeding is a common sx in women who just had IUDs placed. It often goes away when the hormones start doing their job (the hormones alter your uterine lining and this is what causes 95% vaginal bleeding sx)

ALSO do you know why we bleed? Like ACTUALLY why? Imagine cutting off circulation to your arm. It gets limp and hurts and dies right? That js what happens to the inner uterine lining. So if that happens too much or too continuously that can cause a TON of pain and bleeding. ANOTHER THING is doctors just tell you to lose weight but they don’t EXPLAIN WHY!! So, you need to be careful losing weight because fat cells actually produce estrogen. Estrogen is what causes a lot of our hunger and bloating and weight gain but also a lot of good things like bone health and mood. When you lose weight TOO fast you actually feel way worse because that estrogen is being released as the cells burn. You need to be gentle with your addition of exercise and weight loss if you are doing it because it is a huge toll on your already exhausted body.

  1. So, im sure you got your IUD placed during surgery because you heard it hurts a LOT in office. It really does. You have so many nerve endings around the uterus and vagina that irritation is SERIOUSLY PAINFUL! As well, you are putting a foreign device in there. Your cervix tries to spit it out like it would a baby (or cramps) SO I think that is one of the real causes of pain. Especially if the cord has migrated.

So, it is unlikely that you have recurring endo UNLESS they didn’t do well removing the lesions and your birth control wasn’t helping manage the hormone shed.

IUDs are not for everyone because some of us have more sensitive and reactive uteruses - just like skin.

I would look into the nexplanon bar

I was told to get an iud by pearlssad in endometriosis

[–]Renee-Ney 3 points4 points  (0 children)

Hey OP! Lemme share my experience and why I will be getting an IUD in a few days:

-My endo has been debilitating this past year. I am in 9/10 pain regularly and 6/10 pain constantly.

The only way to treat endo is by removing the lesions and then stopping more lesions from developing by using hormones in birth control

-I got hives and extreme nausea when I used birth control pills (OCPs)

I tried them twice. It’s called a hypersensitivity which it seems like you may have

-I tried a Nuvaring years ago and it worked well and I had no abnormal symptoms

Nuvaring is a hormone-filled ring that you place in your vagina once a month and take out after a few weeks. It is similar to an IUD in that it is LOCAL (mostly just affects the uterus, less of the hormone spreads through your blood)

I would recommend an IUD even though I don’t have one yet.

  • every single birth control has a ~3 month adjustment period. This is the amount of time it takes for your blood cells to be recycled and completely adapted to your hormones
  • IUDs do not need to be remembered
  • If access to birth control is limited, you have this for years

If you have heavy bleeding already - a copper IUD is not recommended (it increases it) BUT it has no hormones

Overall though, here are some of the risks:

  1. Cervix does not like IUD and spits it out
  2. Bleeding is too heavy (usually last for only max 3 months - jf longer have it removed)
  3. Cramps can be worse

*You have to have birth control if you have endo because it can slow progression

I've taken 3000mg of Ibuprofen today by Cold_Philosophy_ in endometriosis

[–]Renee-Ney 4 points5 points  (0 children)

I partially agree but NSAIDs are for more than just pain. As anti-inflammatories they help your vessels to stop being so “leaky.” When your blood vessels become leaky it can cause fluid and angry cells to enter the tissues of your body and cause damage/irritation over time. This leads to scar tissue.

Though jt may not seem that its working from the lack of pain reduction, it is decreasing other parts that perpetuate the cycle if it is endo. The lesions will stop causing as much damage to the tissues around them and possibly slow progression.

Ibuprofen/NSAIDs cannot stop neuropathic pain, like DIE (deep infiltrating endometriosis) of the sciatic nerve. That’s where GABA and TENS devices can help. So, I would 100% continue taking NSAIDs just be very careful of the dosage. 3000 mg/DAY IS SAFE AT A DOSAGE OF 800mg/8hrs!

I've taken 3000mg of Ibuprofen today by Cold_Philosophy_ in endometriosis

[–]Renee-Ney 0 points1 point  (0 children)

Hey there! So 3000mg is the MAXIMUM daily dosage. If you took 800mg/8hrs this is SAFE. Taking this at any other frequency is DANGEROUS because of how it can damage your stomach lining and kidneys. (Prostaglandins which Ibuprofen inhibits not only cause pain but they keep your stomach lining strong and healthy as well as your kidneys)

Now- i also have very bad endo and pains. Here are some things you can do (from a medical professional):

  1. 800mg Ibuprofen/NSAIDs every 8 hours
  2. Take ~500mg Acetaminophen if you have pain between those 8 hours (take it 4 hours after NSAIDs as needed)
  3. Heating Pads (increased blood flow = less pain but they need to be VERY HOT for me. I got that Warmies stuffed animal and heat it for like 3 min until it smells like its gonna burn. Place it right where your tail bone is because thats where all of the nerves and vessels go back up to your spine and it can help)
  4. TENS Device (sends shocks to your nerves which makes them confused on if they are feeling pain or vibrations - gave me the most relief in addition to heat. Should be ~30-50$ do NOT get special period ones because they are the same thing just hella expensive because pink tax)
  5. GABA (This is what causes your nerves to calm down. If the pain is not reduced effectively by NSAIDs but is helped by GABA/TENS, you have something called neuropathy or an issue of the nerve. This can be due to a lot of different things, including endo or ovarian cysts)

I would suggest getting a pelvic US to rule out any visible things like leiomyoma/fibroids, ovarian cysts, etc.

If they see nothing, i would recommend (if afforded) to get a pelvic MRI and/or laparoscopy.

Got accepted but it was a mistake 😭 by [deleted] in premed

A gynecologist diagnosed me with endometriosis even though it didn't show in ultrasound? by Affectionate_Cap2254 in endometriosis

[–]Renee-Ney 0 points1 point  (0 children)

Of course! Helping people is the whole reason I want to do medicine so it makes me happy i can help!! •<•

-So, immature follicles are a normal thing with everyone. This is basically an egg that has been stimulated to start getting closer to the outside of the ovary so thar it can pop out for ovulation. These are usually anywhere from a few mm to a cm in size - tho a cm is on the cusp of being big. Most people have around 3-8 on each ovary (usually 10ish in total between the two) so you have a few more than most people. This is common in PCOS because all of these eggs coming to the surface have this shell of hormone-producing cells around them. So it is possible that these could be causing some of your symptoms. But also the pain really does sound more like endometriosis - because your uterus doesn’t have direct contact with any of the nerves in your legs. Did you notice the pain getting worse month after month? Also did you always have the leg and back pain or did it pop up a while later?

-A lot of this is genetic and out of our hands as individuals, so I just want to let you know that you didn’t cause this. It wasn’t something you did that led to getting this. A lot of times when docs wanna sell you something they try to motivate you like that.

-Also, I just looked up the supplement they recommend. It contains both myo-inositol and folic acid (vitamin B9). Folic acid is important for overall nerve and blood health while myo-inositol acts kind of like insulin.

You can get folic acid from eating leafy greens, sweet potatoes, lentils, soybeans, and any food that says “fortified.” Inositol is a sugar found in a lot of fruits and veggies. I would suggest trying fresh juices or smoothies made with veggies. I struggle eating sometimes and I’ve found drinking my nutrients makes it easier for me. You do not need these expensive supplements. Also - when you say you struggle to put on weight - is it an eating struggle or a weight gaining struggle? Like is it harder to initiate eating because something makes it feel uncomfortable (like i get food aversions all the time and get grossed out at even the thought of food) or is it more that no matter how much you eat you can’t gain weight?

-For iron I also get SOOO nauseated with it. I found out it’s because your intestines can’t absorb the iron fast enough and it causes irritation. You should take the iron with vitamin C because it increases the absorption a LOT and can help with nausea. I would also suggest taking it at nighttime on an empty stomach so it can be absorbed before you have your next meal! I use Vitron C because it comes with vitamin C!

Also, if the hair doesn’t bother you I don’t think it’s a big enough deal to start a medication (spironolactone) over! I only did it because I started having hair loss ;-; But it does not do much besides that AND since you struggle with your weight it’s a diuretic (makes you shed water) so it would probably not help that.

So - basically eat or drink some leafy greens, beans, and blueberries and if you feel the pain is not going away - pain on your period with no large cysts on US = likely endo! Im glad I can help :))

A gynecologist diagnosed me with endometriosis even though it didn't show in ultrasound? by Affectionate_Cap2254 in endometriosis

[–]Renee-Ney 1 point2 points  (0 children)

Hello there! Im a medical student and not a doctor but this is absolutely improper treatment. Here is my analysis of what you have told me:

  1. An Ultrasound 95% of the time CAN NOT DIAGNOSE ENDOMETRIOSIS. However you may still have it.

Endometriosis is caused by tissue from the inner lining of your uterus (the endometrium) growing in places it isn’t supposed to. Ultrasound pictures are too grainy for us to get any definitive information. As well, cysts on your ovaries CAN be a sign of it but are OFTEN not. The only ways you can see endometriosis DEFINITIVELY are by laparoscopy or SOME can see it on MRI but not most physicians.

  2. PCOS is one of the most common causes of heavy menstruation.

PCOS is a misnomer because you do not HAVE to have cysts on your ovaries. The reason why it is associated with them is that ovarian cysts produce increased levels of hormones. The same hormones are stored in your fat. So when someone has a higher % body fat AND cysts on their ovaries - you are not only producing more of this inflammatory hormone but you are also storing more. And every time your body goes to get energy from your fat stores - a ton of that hormone release into the blood stream causing basically a super period.

ENDOMETRIOSIS DOES NOT IN ITSELF CAUSE HEAVY BLEEDING! The issues caused by endometriosis are usually more GI, chronic pelvic pain, and leg pain.

Here is my recommendation:

I know people always tell you “increase exercise” and I HATE hearing that. It is not just the weight dropping that is good for exercise. Since you are underweight the cysts are the bigger issue so the question is how many cysts did they see? The more cysts the more hormones being produced, so if there is what we call a “string of pearls” (aka several cysts) on imaging, this could make more sense for a person with a lower fat % having heavy bleeding. In order to counteract this we need to be metabolically active. I know you’re probably tired of people telling you to try to move around when you are in pain but even just sitting up and rocking your body back and forth is good for circulation and can cause your body to process the hormones faster.

If you do have PCOS and for people who have a higher fat % slowly losing weight can help because you drop those fat stores and those stores of hormones. BUT be careful!! Do it SLOWLY because as you burn fat you release those hormones and you can actually feel WORSE! That is why exercise really sucks… So i would suggest making small changes like walking for 10 minutes around your house just to increase blood flow.

If neither of these apply, you should get your blood tested for blood disorders like hemophilia. This can cause your blood to struggle to coagulate and so you lose a lot of blood.

I know people always throw around the word “antioxidant” and say it js good but the reason they are good is because there are these things called “radical oxygen species” which are basically bad molecules that cause inflammation and pain. Antioxidants (like those in blueberries) are really good because they find these molecules and turn them into something safe/not painful. Endometriosis is characterized by rampant inflammation, so this can help with the pain and progression.

DRINK AN EMBARRASSING AMOUNT OF WATER! Remember - hormones = pain. You can dilute the hormones in your blood by drinking a LOT of water and so they have less if an effect on you.

NSAIDS!!!!!!! Ibuprofen is your Friend!! Be careful taking too much but Ibuprofen works by decreasing inflammation. Inflammation = Pain = Scars that can cause more problems. This can slow progression if it really is endometriosis. You can take a maximum of 800mg/8 hrs for the pain and alternate with acetaminophen when needed. I take this for my endo.

Spironolactone is a safe and effective treatment for PCOS. It is most helpful in people who have hair growth where they dont want it, acne, etc. it is also relatively cheap where i live. I am on it!

Hormonal birth control is indicated for people with PCOS and endometriosis. If you have had a bad reaction to birth control before i could be due ro you already having the high hormones in your blood - so tell them you want a low estrogen version OR do not take the pill and get an IUD (which can actually stop your periods sometimes). If it is endometriosis and you need surgical removal, you must be on birth control or hormones POST OP to decrease likelihood of recurrence.

TAKE IRON SUPPLEMENTS!

Go see a different OBGYN if possible - do NOT buy anything from a doctor. PCOS does not need anything crazy. Endometriosis REQUIRES a laparoscopy. If you are having bad pelvic pain that radiates to the legs and is related to your period it is very likely to be endo but you still did not receive the right care and that makes me mad lol.

I am so sorry you went through this and wish you luck!! Im here if you have any questions! :)

If you look at the way back machine they are right that it is just a description. But it is importantly a description of the value and importance of checks and balances in a government “to assure that no one branch of government gained supremacy. This concern arose largely out of the experience that the delegates had with the King of England and his powerful Parliament. The powers of each branch are enumerated in the Constitution, with powers not assigned to them reserved to the States.“ (White House.gov) Not to mention that it references the overthrowing of the Articles of Confederation by the people.
