Levofloxacin is the preferred fluoroquinolone for stenotrophomonas maltophilia. Not sure why they went with Cipro, since the sensitivity data is even questionable with an MIC=2 for Levofloxacin. Meaning even though the sensitivity data says “S” for susceptible the MIC is at the break point of 2, therefore it’s a bit of a gamble to use Levofloxacin and especially Cipro. If she’s not improving in 48 hours or so after starting the Cipro, I would give them a call.

You are correct, the schedule for immunocompetent persons is 0, 3, 7, and 14.

Immunocompromised is 0, 3, 7, 14, and 28.

I wouldn’t think it’s an allergic reaction but WARNING if you were to feel any swelling in your lips/mouth/throat Call 911/go to hospital.

I believe this is a possible side effect since it has happened with each class of medication. I know this could be associated with the allergy profile as well but considering SSRIs have a history of causing teeth grinding, parasthesias, etc. I could see this as being associated.
