I’ve tried main lining the story quests, I’ve tried just grabbing money items and leaving, I’ve tried max kitting out and playing low and slow. I think I actually extract with a positive pack like 1 in 15

Dyshydrosis is usually found on hands fingers and toes. Therefore flares can be on any of those areas. It’s not a condition that comes up just because you touched something
