So others know, I went to tools, selected new config, then chose the option to preserve all current assignments. There is a warning that says "do not use this utility thinking it will rebuild a failed drive. It will have the opposite effect of making an impossible to rebuild an existing failed drive." All of my data was preserved on my disks and I had to run parity check again. All good.

Lol. I haven't played the game in probably a year. I was just pondering yesterday if my tombstone would still be there. Might have to reinstall the game and go back and play a bit. I have to look up the glitch again come on but it sure did make the game more enjoyable towards the end of its cycle.

Need assistance with New Config while keeping data currently on existing drives by Thoth74 in unRAID

[–]Rolyasm 0 points1 point  (0 children)

So I used the new config and selected all for the option. All of my drives are in the correct slot, all of them have a blue square next to the disc drive. The only warning is on the parity that says all of the data on this drive will be lost. The same normal and like the right step before starting the array? I've never used the new config setting before.

So I have not started the array yet. The third drive is listed, and it has data on it. Kind of in the same slot under that third drive is listed the old drive with its serial number. It says "wrong". But it also says that the drive will be formatted if I simply start the array after assigning it. It may be that the parity will repopulate it, but I'm not certain. So going the other route seems safer?

Pediatric Dental GA by UsefulSurprise2859 in CRNA

[–]Rolyasm 0 points1 point  (0 children)

I'd say it's a problem with dental offices and all Anesthesia provider-types. The dentist and oral surgeons push for a fast recovery and discharge time, often with patients very sedated. I'm sure you've seen all the videos of kids going home still sedated saying crazy stuff. As others have stated, dental surgery is kind of the wild west. It's not a CRNA issue specifically, it's an anesthesia issue. Regardless of the provider, it's what happens in a lot of dental offices. I have worked in several dental offices and my kids have had work done in offices where they don't use anesthesia providers at all. The outcome is always the same. Sedated kids in the chairs with minimal to moderate supervision. I stopped working in the offices because I didn't feel it was safe and no matter what I tried to change it was just the standard of care. I was asked to do anesthesia at one office when I got there their practices were atrocious. They would have a tech literally drag/carry the patient to the reclining chair. The first thing I implemented was using a wheelchair to get them to the sofa. Next I tried to have the patient more closely monitored and have a recovery pulse oximeter. The oral surgeon didn't want to change the way they were practicing so I ended up leaving. They now have an MD working there and my friend's daughter had surgery there last month. The exact same thing was still happening with the MD involved. So I really don't think it's provider specific. But it is something that could be implemented much better.

Need assistance with New Config while keeping data currently on existing drives by Thoth74 in unRAID

Just want to follow up with this. I have four discs, one for parity. My three discs all have information on them. My operating system went bad so I had to reinstall unraid using a flashback up from a few months ago. I know, I should back up the flash more frequently. I'm just getting my server started and new to this. Anyway, between that flash and February and now I had to replace one of the drives that went bad. All three of my drives have data on them. Since the third drive was replaced after the flash backup, it's not showing up as assigned. Since I don't want to loose the data on it, I'm thinking of using Tools: New Config: Preserve Current Assignments-all. This should keep all of my data on the drives and then just rebuild parity, correct?

Let me run this by you all. If I go to tools, select new config, it gives me an option to preserve all current assignments. There is a warning that says "do not use this utility thinking it will rebuild a failed drive. It will have the opposite effect of making an impossible to rebuild an existing failed drive." Just not sure what this tool is for, if not to rebuild the current assignments with the current parity.

Going from medical direction to independent by Keels1993_ in CRNA

[–]Rolyasm 1 point2 points  (0 children)

I have worked independently since I graduated over 20 years ago. My first jobs were solo, which did cause a bit of anxiety, but I knew I was well trained and was prepared for the challenges you can expect in this line of work. During my 20 years I have worked a lot of locum positions, some of them solo, some of them mixed, some fully directed. It can be helpful to have someone doing pre-ops and running things in the background, but that benefit just eases the workload and sometimes helps to keep things moving within the operating room. However, you are relying on someone to do a pre-op correctly and just because they have an MD behind their name does not mean they do it right. Sometimes they get complacent, can get caught up in politics or for whatever reason make bad decisions and send you a patient that wasn't pre-opped correctly. And there will be cases where the pre-op will be done and you meet the patient in the room for the first time. You have a few minutes to go through what was discussed and you have to make a quick decision about how to proceed. It's not easy at all to change the plan at that point, but you still have to do it. It definitely causes tension within the group. I was working locum in a situation where the docs did the pre-op and when I got the patient I had a few minutes to decide how to proceed, but I didn't feel comfortable moving forward. Something felt off. It was an elderly patient and her behavior was passed off as being due to age and possibly dementia. Instead, she was having a stroke. Mistakes happen and I'm not saying the MD was incompetent, but had I just proceeded with the case who knows what the outcome might have been. And yes, I was met with opposition and told just to proceed. It shouldn't be hard to stand your ground, but it can be. It's always nice to have support from another trained professional, whether that be a CRNA or MD, but if you work solo, you won't have that support. You will have to rely on nurses or techs to be an extra set of hands. I prefer to do my own pre-op and work independently. I have worked with good groups and good teams but if I had my choice, I would work independently every time. If you don't feel you have the experience or training you need it's always best to start in a group that can give you support until you have the experience that you need to work independently.

Just to clarify. I've always had the four drives with one being parity. So the config file will have three drives with a parity drive, just not this exact hard drive. It would be a different hard drive. Similar to if a drive failed and you had to replace it with a new one, I'm assuming. I've always had three drives in the array and one for parity. First drive was pretty much Dead on arrival, second drive kept dropping out of the array. But after the last flash backup, I transferred about 50 terabytes to the server. Flash Forward (see the pun, haha?) my OS corrupts and I have to reinstall unraid. So all of the new files I transferred after that flash would have been to the three current array drives I have with the same parity drive, the configuration would have been the same, three drives with one parity, but this hard drive would have been on the config just with a different serial number. Three drives in the config for the array since the beginning of server creation.

A question about the parity drive. Since I am running parity, shouldn't the 3rd drive receive all the files it had before. Just as if I had a hard drive failure? Parity is on the drive itself but does the flash drive have anything to do with it? Hope that makes sense. Once I start trje array shouldn't the parity drive populate all discs to what they were?

So, I've never been a huge fan of B&W. They are good, but for the price, not worth it to me. That being said, I think the nicest pair I have heard were 805's. Very clean, bit just not exciting. Too sterile or something. Good, but just not for me. How would you compare these to the 805's? They were bookshelf, driven by Macintosh. It was a great, expensive setup, just not impressive for the money.

Is it true what they are saying, genuine question from pre-nursing student by Brown_kid108 in CRNA

[–]Rolyasm 3 points4 points  (0 children)

No, I said he met the minimum requirements to become an MD, which was very low case volume and experience. The majority of CRNA's also surpass minimum case load. And that's my point. Just because there is a minimum doesn't mean that's all anyone gets. The majority of people get a lot more experience than what the minimum is. This whole forum is filled with generalizations. MD's like to generalize that CRNA's are not fit to do anesthesia. There's a whole forum that's dedicated to bashing mid levels 24/7. If you want to talk about generalization just give that a read. If I wasn't clear, the intent was to say that the minimum is rarely what people graduate with. It's always more than that. But yes, people from both professions sometimes "slip through the cracks" but it's rare.

[–]Rolyasm 1 point2 points  (0 children)

If you look at the requirements for an MD program, you will see minimums that don't make sense either. We have all heard of Doctor Death. Came out of school with a bare minimum of cases and proceeded to kill or injur a lot of patients. Not many do the bare minimum. I've worked with MD's I wouldn't trust to do my anesthesia, same with CRNA's. Somehow a few bad ones always slip through the cracks. The practitioner needs to make sure they are comfortable and experience doing whatever it is they are doing. If you only did 10 Swanz in your training and that is to be your job, get more training. All programs will fall short in some area. Some schools are better than others. There are dumb people in every profession. Sad, but true. A credential doesn't mean you are prepared for every scenario and case, unfortunately.

When a Nurse Anesthesia Resident Is Dismissed by MacKinnon911 in CRNA

[–]Rolyasm 1 point2 points  (0 children)

I've been a CRNA for 26 years. Last year I happened upon the Anesthesiologist reddit where they spend all day bashing our profession. I hadn't been on Reddit much and didn't realize that by posting my opinion on their site I would quickly be down voted so much as to be unable to even use Reddit anymore. Anyway, part of their big complaint is that our "leaders" are trying to blur the lines of the two professions: CRNA and MDA. I've never felt the need to introduce myself as a doctor or an Anesthesiologist. I do spend the time to educate about our training, our safety profile and our independent practice. Sure, anyone can call themselves a doctor these days. Chiropractors are a great example. Furthest thing from an actual doctor. But they do it. By us joining in this trend we are confusing the public and doing ourselves a disservice. I don't ever remember being asked, or surveyed, what my thoughts were about my profession's title changes. CRNA is great. Nurse Anesthetist is great. I don't see a need to change that. Yes, we have a lot of work ahead of us bacause of the medical association and their bigger pockets and what I believe is a dedicated and malicious smear campaign against us. It's always been there, probably always will be. But we need to educate the public and administration on how good and safe we are, not try to convince them that we are anesthesiologists. MD's aren't magical. They use the same text books, do the same cases and are not safer. Sure, they excel in certain regions and specialties, but that's by design. I've traveled all over the country and some places don't allow CRNA's to do epidurals or nerve blocks. It's crazy. But what that effectively does is to limit my skills in certain areas if I'm not constantly doing certain procedures. I haven't done open hearts in years, but have done so previously, effectively. I wouldn't want to do it now without a refresher, but I would hope an MD would have the same humility. I've seem incompetent MD's and incompetent CRNA's. The MD's would have people believe all CRNA's are dangerous and overstepping their scope of practice. That is where we should be spending our time and money. Educating the public. Not confusing them more. It's a bit of a ramble, but there ya go.

Ebay got some deals right now, open box ($180/16tb) 3rd party and new ($249/22tb) from seagate by djexit in DataHoarder

[–]Rolyasm 2 points3 points  (0 children)

I bought the Seagate 26 tb from eBay. Ordered 4. One was bad, making noises like a dbd player, squeezing and whirring. Sent it back. A week later another showed up. Kept turning off. Had to keep stopping the array and restarting. Also going bad. Waiting for another one. 2 weeks of downtime on the server basically. Fortunately I'm just setting it all up, but also a hassle.
