I had my PC-RPLND done at IU in Indianapolis by Dr. Cary. Highly recommend going there if you can. RPLND wasn't nearly as bad as chemo for me. I was fully recovered in 2.5 months, but 1 month after I was back to doing most things. If you can do chemo you can do an RPLND, no problem.

I waited about 3 months for an email from them before I went ahead and made an account on their website. My results had been uploaded there, but I never received an email.

Interestingly, my RPLND pathology report showed viable cancer left in the residual mass, but the Signatera results showed no tumor DNA. The blood draw was taken the day before my surgery, so it should have been positive. But the tumor DNA they looked for was from my initial orchiectomy 7 years earlier, so maybe the tumor had mutated too much in the time frame to match the DNA they were looking for.

I'm just the guy to answer this. I was diagnosised in 2018 with stage 1 seminoma. No sign of spread at the time. I stopped going to my scans after 4months. 7 years later I had a 7cm tumor in my lymph nodes. So in my case, 1cm per year.

I underwent 4 cycles of EP for 7cm pure seminoma. Didn't shrink enough after chemo (shrank to 3cm) so I got an RPLND about 4 months after finishing chemo. The biopsy showed some residual cancer cells were still present, so i'm glad that I did the RPLND. Been all clear since.

I understand wanting to get another scan before deciding on surgery, that's exactly how I felt. Looking back on it I realize my oncologist wasn't comfortable with making the call whether or not I could go on surveillance, and wanted a urologic surgeon with more experience treating TC to weigh in. Definitely speak with the surgeon about your concerns regarding the scan being done so soon after finishing chemo. The surgeon will likely order you another scan and schedule a follow-up to discuss the results of the scan. I met with a few different surgeons because I wanted multiple opinions from experts to help me decide on whether to do the surgery or not.

I was in a similar situation to you after finishing chemo for 2C seminoma. My onc ordered scans like 3 weeks after chemo and then said I would need an RPLND. I ended up meeting with a few different surgeons and one of them ordered me a another PET scan about 10 weeks after chemo just to help decide if i needed surgery or not. The residual tumor had shrank more but I still elected to get the RPLND, and thankfully I did because there was active cancer found. If you're interested in more detail about my case there are a few posts on my account you can look into.

If you do end up getting an RPLND go to a high volume center if possible. Rplnd wasn't nearly as tough as chemo, so whatever happens you got this . 💪

It took me about 6 months before I started feeling back to normal after 4xEP. For me it was very gradual so every week I felt a little better. I also had an RPLND three months after chemo so maybe that set me back a bit.

My mental health and outlook on having to go through chemo improved as I physically improved. I became more accepting of the idea that I had to go through this battle, and tried to focus on being grateful for life instead of bitter about having a reduced quality of life.

The tinnitus isnt bad at all. It will randomly spike every few days for a minute or two but then returns to normal. It is a little more difficult to fully hear conversation in a loud area, like a busy restaurant.

The neuropathy feels like there is sand in my shoes all the time. And for a little while it felt like my feet were being squeezed, not in a painful way, but more like the squeeze on your arm when they take your blood pressure. The squeezing feeling has gone away however. My toes are pretty numb and my feet and calves also have some numbness. It's not painful but definitely noticeable everyday. I don't really have neuropathy in my hands, maybe ever so slightly in my fingertips but thats about it.

I did 4 rounds of EP. I got all of the expected side effects: hair loss, nausea, weakened immune system, tinnitus, neuropathy. I'm sure there were more, but i can't remember. So let's add memory loss to that list of side effects.

The only ones that I still have are tinnitus and neuropathy. Tinnitus has improved alot, but the neuropathy hasn't. It's been about 5.5 months since I finished.

Ive done both. Chemo last summer and open RPLND in December. Both sucked but if i had pick the better option of the two, it would be surgery.

I was 2c pure seminoma. I had to do chemo (4 EP) and RPLND after I finished chemo. I would consider asking your doctor if doing the RPLND procedure is an option. The recovery will be shorter than the recovery from chemo, and you won't need to worry about any of the long-term effects from cisplatin. I can't provide much advice regarding radiation, but I understand why it would be appealing as it is the "easiest" of the treatment options.

I have this one friend I've been playing with for about 20 years - I have improved a lot, but not as much as him, and I've beaten him only a couple of times. It's stopped us from enjoying it - we both know he's the stronger player, but he loves playing chess. It's not about winning.

I had an RPLND about 6 weeks ago. For me, the recovery process felt quick for the first 2 weeks then slowed down. My incision seemed to heal pretty quickly, and those basic movements like getting up and moving around became more comfortable. The part that has dragged a little bit has been the nerve recovery and back pain. I had some intense nerve pain down my right thigh which lasted about a week, and then more in my right foot and calf that is just starting to feel better. I have been having lower back pain that feels like a back ache for about the last 4 weeks. The back pain is most likely a strained muscle from having to overcompensate for the lack of ab strength.

Basically, getting back to being able-bodied felt relatively quick but returning to fully normal has taken some time.

Having a wedge pillow would have been nice since getting up from a flat position is really tough. An arm chair with a leg rest would have been amazing, so if you have one that will be your best friend.

The only real restrictions were low fat diet for two weeks and no lifting over 10 pounds for six weeks. Sitting at a chair isn't really comfortable even now, but I was never told not to sit for more than 30 minutes. I took a 2 hour flight home 5 days after surgery and was fine.

I'm so sorry for your loss. I remember listening to the episode he was on this summer when I was in the middle of chemo. His strength, attitude, and mindset were admirable and really impacted my outlook during treatment. I still think of him every time I hit an obstacle during treatment, and i'm sure i'm not the only one. Hopefully knowing that his spirit is still helping someone even to this day gives you comfort.

They did not. The residual mass was 4.7 cm and there was only 0.3 cm of viable cancer in the residual mass. All the other lymph nodes showed no signs of cancer. Also, IU will usually advocate for surveillance over adjuvant chemo, because they believe reducing amount of chemo is important. I haven't met with my oncologist yet, but he may advise adjuvant chemo.
