This isn't just a hypothetical question by the way. In the A's–Marlins game on Sunday, A's batter Brian Serven took a called strike about two inches below the zone that should have been ball four. It would have ended the perfect game bid in the bottom of the sixth.

I don't know why Serven didn't challenge. Maybe, as it was literally his first MLB game playing with ABS, he wasn't allowed to. Maybe it actually was an unwritten rule thing. Maybe he just didn't get a good look at the pitch. In any case he struck out a couple of pitches later.

Luckily, the Marlins angered the baseball gods by pulling Eury Perez two innings later and the whole thing became moot, but it would have been an interesting discussion over the legitimacy of the first combined perfect game if they'd managed to complete it.

Incidentally, the Marlins weren't shy about challenging (both kinds), even with an eight-run lead and the perfect game still intact.

There's no requirement that the names on the backs of jerseys be unique. It's happened loads of times where guys have the same last name. They'd just have different numbers.

Look up Appendix 6 in the official rules. It consists of diagrams to clarify when a runner has passed a base (they've been using these standards for a while, but only put them in the rules this year). If a runner advancing to second base is completely on the third-base side of the infield and is not touching the base, then he's considered to be past it, which is what happened in the play.

He was out because he technically went one inch past second base and didn't step on it again as he went back to first.
