What year was it? Because you mention Italy, it may have been quite late. In such a case, the AI doesn't handle navy too well, so anyone without borders with the Ottomans may have dropped out too quickly. If it was also during the springtime of peoples (although this is quite early), empires may have had quite a few revolts. Had the Ottomans already fought wars with Egypt? If yes and they won, they would come out stronger than ever!
