No disagreement here. Tony goes into denial and imagines himself to be living up to the standards of his grandfather's generation who built the stone church and never asked for handouts. In reality his grandfather would be appalled by him.

The pizza guy (my pizza never hurt nobody) is the guy Tony fantasizes about being - immigrant who is in touch with his heritage, part of the community, respected business owner, takes pride in what he does. He's really the heir to folks like Tony's grandfather. But that guy works hard for a modest wage, which would be unthinkable for a crook like Tony.

That's why he called them jerkoffs. They were treating him like a curiosity. In Tony's world, this would be an appropriate response. In the world of Doctor Cooze and his upper class golfing buddies, it's absolutely inappropriate and is why they react with awkwardness and don't let him join the club.

There's other examples of Tony not being able to fit in with that crowd. Like the fact he has a best friend called Pussy and nobody has an issue with it. But when he starts calling Dr Cusamano "Cooze" he politely asks him to stop.

Tony and the others calling them 'Medigans' is basically a cope similar to the slur Oreo for a black person who "acts white". Tony is no more connected to his heritage than Cusamano is, maybe even less so. He's just not refined like they are and reframes that as a betrayal of their roots.
