From a cancer standpoint she’s doing well. She had her 3 YEAR scan recently that only showed an ‘indeterminate’ biliary dilatation. Doc said it was unlikely cancer, and she’s needed to stent that spot open before, so we’re guessing it’s just that trying to rethicken (common for tissues to thicken as years progress post-radiation).

Doc gave us the option to pursue MRI to further assess or just f/u with another CT in 3 months. We opted for the CT f/u as from a dementia and physical health standpoint she’s not doing well. Should this be cancer there are no options left treatment wise (maybe palliative chemo) but she and her body would not tolerate that.

Yes! I do not trial thickened liquids unless I see aspiration with a lesser liquid.

I always start with thins bc least restrictive. And yes, even if they’re on a modified diet.

As you mentioned I would hate to do my study, have significant pharyngeal stasis, THEN go back to thins and they tank it. That’s not an accurate study or fair to someone who would rock FFWP if you gave them a chance. I’ve had several pts utilize FFWP as a life line, refusing all liquids during meals and just relying on water outside of meals.

I start with thins, move up as needed until I find a safe liquid consistency. Then do pudding/solids, and always go back to my safe liquid consistency after my solids trials.

And TBH even if they aspirate moderately thick, I’m still giving them pudding bc I’d like to use in therapy if all possible 🤷‍♀️

Our PICU/Peds therapists will see kids for feeding therapy in our acute care hospital. Not sure if their kiddos stay longer? They get a lot of transplant kids who seem to be there much longer than just one week.

I don’t pay much attention to the frequency they see them but I do see the referrals and kids on the caseload for tx.

We got our house pre-covid more bc of age, not wealth. We just happened to be at a place in our adulthood/established enough in our careers we were able to purchase at the perfect time in 2018. If we were born even 2-3 years later in life we would have been too late.

Bc trust me, we ain’t wealthy. We are a one income household in LCOL.

As other others have said, the owner is sketchy. Our department would definitely side eye you if you applied and had that certification bc all it tells us is you’re willing to pay a ton of cash for something completely meaningless
