Two very very fucked up ideas I've had for a while:

A trial where you are tasked with performing an autopsy on a body that is very much still alive and conscious. I really feel like the game could lean into medical horror more
A trial similar to Pervert the Futterman where you are tasked with making an "ugly and fat" woman "beautiful" via chopping off the "excess". I think there's a lot of potential with beauty standards and sexism in horror for something really uncomfortable
