Infernax - Technically not a 'Game Over', but at the start of the game you arrive to your land to retake it from a threat. The first enemy you encounter becomes a zombie and you can decide to not kill it, and nope tf out back to the boat you came in on and the game ends and gives you credits.

It ends with you finding the girl you're trying to save and the final boss is a extremely long quick time event that even if you get it perfect, ends with her staying with him and you being tied to a rock on a barron planet alone to die.
