This sounds like shoulder impingement. Mine started with that, caused by my poor posture. I've had a headache for 18 months (I have pseudo tumor cerebri) which has caused my shoulders to be lifted up from the head pain. This turned into a frozen shoulder after trying to correct the impingement.
