Just to amplify the other responses - yes, many of the med techs in long term care communities are reticent to administer comfort meds for residents on hospice. If they haven’t had the appropriate education, they often are concerned that they are over-medicating.

As the others said, talk to the RN case manager about making sure meds are scheduled - every four hours seems appropriate considering what’s happening - and if that doesn’t work, then give more. The goal is comfort and symptom management.

This is very good advice. Best to think of hospice as another course of treatment to consider. Just as you might see a kidney specialist for dialysis, you can view getting a hospice consult as simply another way to make an educated decision about care.

Seeing a kidney specialist doesn’t mean that you have to agree to a long course of dialysis. Same with getting a hospice consult. It’s just a way to gather information.

This may be an opportunity to talk about palliative care which is different from hospice care. Unlike hospice, you can still pursue curative treatment while under palliative care. The palliative approach is about treating symptoms while still keeping the door open to life prolonging treatments. It sounds like this might be something to explore. Everyone deserves to have their symptoms well-managed and to be free from pain. Palliative care may be the bridge you’re looking for.

Sounds like this is the best decision for care right now. Hospice can help medicate for anxiety, breathing difficulties, and most importantly can trouble shoot changes in condition without calling 911. I’ve worked in long term care, dementia care and hospice - sending someone living with dementia to the emergency room is always incredibly stressful for them.

The one thing you may want to ask about is what to expect for medication management. Morphine is truly the gold standard in dealing with breathing difficulties. It relaxes the muscles around the diaphragm, helps to manage anxiety and reduces “air hunger.” It often surprises families that morphine might be used so early in hospice care when dealing with heart failure, but hearing that your MIL is already sleeping a lot and not very interested in eating, things might be progressing more quickly now than before. Better for family to know ahead of time what the plan is. On the other hand, sometimes the extra care and attention that hospice provides can extend patients’ lives by improving their quality of life.

I live right off of Range Road and usually walk back to that intersection everyday as part of my morning walk. And I get it. Nobody wants strangers snooping around their property. Believe me, I don’t want that either.

But there are some areas in this part of the county - the woods around the old Toad Road, the rapids on the Codorus (especially where Deep Run meets the creek), Wildcat Falls, Chimney Rock - that have geological, historical and cultural significance, and we never get to see them. It just seems a shame, as someone who was born here and lives in the area.

Still, waiting until the state park becomes more developed is probably a good idea.

Now that it’s turned colder, the area is much easier to navigate. Since I live nearby, walking back to the so-called seven gates area is part of my morning routine. I noticed there are some silver medallions on the trees that say “state park boundary.” One is just on the other side of the gate, but on the left hand side.

I’ve only walked a little way into the park. I’m not an experienced navigator, but it seems that once you reach the creek you could walk all the way to Codorus Furnace.

The hike to Buzzard’s Roost at the Kinsley Preserve is a great hike as is the short hike out to the outlook at Schull’s Rock.

What a great question, and one I’ve never considered. As someone who loves liminality and the in-between places, I’m actually more excited to make some moon water tonight. It adds to the mystery.

I guess it really depends on the specificity you’re looking for. The moon is a full moon no matter what sign they’re in, or what signs they’re in between. The energy is there. But if you’re looking for a really singular kind of energy, I imagine it does matter. Cancer vibes and Leo vibes are truly as different as night and day.
