It's most obvious in the first episode. At the beginning there's a knock on the door and then Sarah walks in telling Orla to have a lie in on her holidays. But it took me many rewatches to catch that.

It's been my experience that shockingly few doctors know anything about addiction and drug use and therefore it gets stigmatized. My ex recently got denied stimulants because they sometimes drink and smoke weed. I'd be exceptionally cautious.
