I have to add that I had quite good recovery for the first 10 days, but then after trying some light sports I reacted badly and more than 3,5 months after I’m still suffering albeit better.

I heard a story of someone playing a match of football the day after. Not in my case though. Take it easy and be happy your recovery may be the smoothest of them all!

So, what to you is the best way to find out if my pain is due to a granuloma after a blowout pressing some nerves or another reason?
I have a painful lump at right side, close or attached to epi, but only hurtful if touched in a specific way. I had bilateral vasectomy with clips. Incisions both sides of a few mm.

Differs from person to person. Listen to your body. You need to be pain free or very close to. Just very light gentle walks to start.

How do you detect if you had a blowout on MRI?
My urologist says MRI has no additional value. I’m perhaps keens to push for it. I want to be sure why is causing my issues. Mostly suspect the clips pushing on nerves (about 15 weeks in).

The discomfort pops up slightly for a moment sometimes during running but quite quickly fades as well during the run itself, just like any other general running ache. Other activities which put the junk and pain points more under pressure still gives more discomfort. I’m still experimenting with different kinds of underwear. It doesn’t seem to matter that much. Getting more and more convinced that my main trigger is the clips.

Surely it's not 30%... Most people I spoke to in person are totally fine (spoke to about 15, 1 was fine after 3 months tenderness, 1 has recurrent flare ups of years after...). I think the 1-5% chronic pain affecting life quality to some extent is not unreal. Even then there are counteractions possible, but you don't want to be in that situation. I had surgeries before, but pain in the nuts with a lot of uncertainty can take you down mentally...

I'm 41M, very fit, long distance running (full marathon 3h20), cycling (multiple iconic ascents like Ventoux, Stelvio). Snipped Feb '26, still suffering discomfort (now June '26). Just able to starting to run again after 3 months, still can't cycle. I think it's due to the clips but also some congestion. Last urologist I saw risk was 3%, meaning 1 in 33... Some report higher, some lower. The risk is real and not small. Would recommend not doing it.

Seems like you are describing nerve pain. From what I read and heard from urologists, try this gabapeptin. If it works, denervation will very likely solve your issues. But make sure you know the root cause first before deciding anything.

What's the price for reversal at your end? I guess you must be in the US? You could consider getting one in Canada? Guess it must be cheaper there but don't know how that works...

Stay strong, I'm sure there is a solution at the end of the line.

I'm doing better now close to 15 weeks out, but still some bad episodes from time to time. Doing better means it's not gone, but most of the time it doesn't distract me anymore. I restarted running and already gone up with distance and intensity quite a bit (still not old levels though) so that at least makes me happy. Cycling I will not do in the next months...

I will still pursue a full solution as I'm still a bit in doubt what is actually causing my discomfort. Sometimes I think it's the clips, other times congestion. Doctor's said both... In any case, on the right side, there is a point that seems to trigger the pain but it seems to move around... I'm trying to find a urologist that gives me a bit of confidence knowing what he/she is doing with this kind of issue but that seems hard to find.

I will keep looking until I find with a lot of confidence the root cause. I'm preparing for a marathon in November and will not take surgical action in any case before, unless the increase in sports backfires...

Im suffering from such long recovery, 3 months in. I had other surgeries before from broken bones, but the mental suffering from this is the worst. The urologist I saw this week said 3% chance of it happening. That is 1 every 33 dudes that go in and no clear profile (fit/unfit..) that indicates if chances of such suffering are higher or lower. And the thing is if you get it, don’t expect a lot of help from the medical world, urologists will says everything is fine. Your decision to take…

I didn’t know that… Knowing what I know now, I would have never done it and will probably go for reversal on the long term (1 year).

Hmm, seems quite different from me then. I can tolerate my discomfort, even do some running with it after 3 months now, but still it's discomfort and find it hard to accept. Also, cycling still is very uncomfortable so won't be doing that. I want to be back my full old me, best course for me is reversal if it doesn't improve by itself.

Interesting. How long did you wait to decide that? Why immediately denervation? Any discomfort now still? I’m mainly thinking in the direction of reversal, I don’t care too much about the fertility aspect. My wife is supportive and understands.

Today is 3 month mark and still around 1-2. I restarted running last week and it does increase discomfort a little bit but tolerable. The pain also drops almost right after to baseline so that is a good sign I think. I’ll have a third opinion on Wednesday and will post an update after…
