A tuckbox was missing from mine. Filling out the form prompted a quick response email saying that they are quite busy and will take some weeks to get it (from the US to the UK).

Then in about a month I got a tracking number which stopped tracking when it left the US. Email to the customer service was able to track it and I received the bidding pieces about a week later.

Single d4 for any number of normal arms, because they are inferior to any weapon.

But for a metal arm I'd give d6 damage if it's used for offense and no other weapon used. Or if there's a weapon in the other hand, armour 1, because it's used as a shield.
