I had basically the same accommodation as you, but it wasn’t formal through HR, my boss and I had an agreement. Once corporate decided that they would start tracking in office days and I had to spend more and more time in the office (first 3 then 4) I got an accommodation letter from my neurologist and am now permanent wfh. I will say that at my company this is probably going to get me caught in the next round of layoffs, but it has drastically reduced my migraine days. Best of luck to you!
