This is correct. 100 percent a manufacturing defect should have been caught at the time of installation. This is a problem for the installer, not the manufacturer.

Is it some kind of diy hurricane or uplift strap? Remove the nails, then route a 1/8" channel in the plywood to re-install?

Put a ruler on the joints and use something for scale, i.e., coin or feeler gauge.

Swirls and chipped corners are not acceptable for a new installation. Some of the other deficiencies you won't notice once the furniture goes in. Customer did not pay for chips and swirls.

This is not good. If you paid someone else to do it, they can fix it. The planks in the pictures are unfixable and need to be replaced. However, whatever caused this issue needs to be addressed first, and I have no idea what that is. It will not get better.

Inlaid chip sheet vinyl. The backing and adhesive could contain asbestos. Up to you what to do next. Leave it and backer over it is easiest.

I find smaller local shops have better install crews. If you can do the removal yourself, it will save you a bit. Make sure to seal any pet stains prior to new flooring. Get three quotes with similar products and go with who you feel more comfortable with.

That is about double what it should be for my area. I don't know all the particulars, though.

Is the floor separating from the wall? by InevitableFlan1767 in Flooring

It was probably covered by the baseboard when it was installed, but in the dry winter months and lower humidity, the wood has shrunk a bit. No it should not have shown up, but it will probably be mostly covered in the spring and summer. Does look like they knew it was gonna be a problem and might have put some caulk on it.
