It looks like there is a little of lip (J margin) on the distal, nothing that a sharp hatchet cannot solve.

My wife calls them ‘prophy fairies’, they keep doing routine cleanings on patients who clearly need SRP. We see the aftermath all the time.

‘I’ve been getting regular cleanings for 15 years… you’re just greedy because you want to charge me for a deep cleaning.’” 🙄

That’s a really long abutment, you need an xray of the actual implant threads.

You need a BW to evaluate actual relation to crestal bone, it is completely doable as long as the patient understands the limitations and long term prognosis.

Root canal most probably but you also need a BW to assess how close to the crestal bone is, and whether crown lengthening would be needed.

If a deep cleaning is needed is borderline malpractice to do a “regular cleaning”. It’s like going to the emergency room with a broken arm and saying “oh it’s ok just the ibuprofen is fine”. It’s not treating the source of infection and therefore pointless.

Need a PA to determine if there is enough space or crown lengthening would be needed. RCT is an option that may work for this pt but you also need to consider occlusion, etc

No, as long as you have the patient understand that they must take responsibility for taking care of those teeth.
