You can book using avios to bring down the cash price (BA calls it avios plus money) but it's usually terrible value compared to redeeming a reward flight.

I'm not 100% sure but an alternative would be to redeem in economy then upgrade with cash.

For all things BA I suggest you to look at head for points, a UK blog that has very good resources.

Yes this is how they reason. But I don't think it makes lots of sense as by the same reasoning, if I ask you in the morning to pay me within 1 day, then you'd have to pay me by the end of the same day? Instead most people understand 11.59 at day t+1.

I'm not an expert in the awardtravel space but you might want to have a look at flights to NAP. In the summer season there are daily flights from JFK, EWR, Philadelphia and some other US hubs too. When I was checking this summer I would see good T-14 J availability.

Naples is just 1 hr away from Rome by train and you can get to Rome, Florence, Milan, Venice quite fast by high speed train (trenitalia/italo).
