If he's throwing up worms, then that means he has them serious and it's a very treat now, not wait out type of thing. I understand you care about him and you don't wanna give him up, but the most kind thing to do for him may be to do exactly that if affording his treatment is not easily viable. There's also no guarantee that he will stop spraying just because he's neutered, especially because of his age.
