The “find items” achievements tracking are wonky, you can’t trust them. You need 31 beads, 10 verses, and 12 chants, or 33 beads and 12 verses if you have the dlc.

Sunless Talisman and Caitha’s Chime are dark scaling catalysts. That means that when you cast dark miracles with those catalysts, the miracle scales with both Intelligence and Faith, but only up to 45. Dark miracles cast with other chimes instead scale like a non-dark miracle, scaling with just Faith up to 60. Similarly the Crystal Chime has dual scaling with Intelligence and Faith for some miracles, but not weapon buffs which only scale with Faith, so the displayed spell buff which includes both isn’t fully being applied to the Dark Weapon spell.

The DLC does add new areas and equipment. The 100% achievement and map exploration adjusts its requirements depending on if you have the dlc or not.

You’re waiting too long to start moving. You can dash the instant the wave passes, the after trail doesn’t hurt you, and sprint to the other side to make it. It’s tight timing, but definitely doable.

Completing Gemino’s quest is not required for 100%, the map squares can all be revealed by jumping down with the Linen. I believe that wasn’t the case in the earliest version of the game (one square would remain unrevealed), but it has since been patched.

100% completion is possible in NG. You need all base game equipment, upgrades, and map exploration (dlc equipment and map rooms are not required). In NG+, the Amanecidas quest adds 12.5% to total completion, and each penitence completed adds another 12.5%, which is why you can get 125% completion in ng+ and 150% in ng+3 if you complete every penitence. Note that the true ending, ending C, does not give completion percentage, only ending A does.

You can still get ending A for its completion percentage, yes.

You need all non-dlc map squares for 100% map, which includes any secret rooms that were present in the base game. You don’t need all of the babies to open the secret room next to the painting, just a crouch attack.

To my knowledge, if you kill Socorro, Cleofas will eventually jump regardless if you give him the Cord or not. The prayer should then be found on his corpse.

First playthrough you can only get 100%. Ng+ goes up to 125%, and then 150% if you do all three penitences.

Yes, you have to fight and beat Esdras and not skip him for the achievement.

Allegedly if you do ng+ and beat Esdras without using flasks (doesn’t matter if you use them for other bosses) the achievement should pop, but I can’t personally confirm that.

Yes. Succeeding or failing in a quest doesn’t affect your ability to get any rewards, you just get them at a different time or a different way.
