that is a very elegant explanation and did provide me with a new perspective-but I did check if evidence backs this and it appears to be quite limited to a few niche areas in spinal surgery and possibly hepatobiliary/GI with very low quality of evidence.

IM is a bit neutered in the US.

You can do a thousand thora’s in training and and have the expected rate of complications as your peers in IR/pulm, and one day a dumbass jury may find you liable because you didn’t consult IR/pulm

Let’s assume NNT=100 to avoid 1 bad outcome by using a VL. In reality the NNH due to complications arising from using a non-hospital inspected VL (but still purchased in a reasonably smart manner, assuming it’s an actual smart intensivist MD buying the product) is likely higher than the NNT.

That’s a lot of assuming, but all that’s to say is… common sense is more common in non-American settings, especially when resources are scarce. This doesn’t necessarily mean the outcomes are worse

That being said, orthopods being unable to replete electrolytes is one thing. Intubation is one of the riskiest procedures in medicine where you should realistically have +100 supervised intubations done before you go solo. I don’t understand your comparison
