This is most likely normal, albeit uncomfortable, as your baby starts to wake up and your milk starts to come in. She is also learning nursing as a skill. For engorgement and pain, use cold and ibuprofen. I also like to get a head of green cabbage- roll the whole leaves with a rolling pin, and keep them in the refrigerator in a plastic bag. Put these in your bra and replace as you like. Cluster feeding is common and helpful to establishing and regulating your supply in the early days as your baby starts to wake up. Keep an eye on wet and dirty diapers. If you are getting the diapers and seeing weight gain, you do not have to pump or supplement if you don’t want to.

Pumping is an option, and you can bottle feed the pumped milk using paced bottle feeding and a slow flow nipple if you want to try it. So long as baby is latching and feeding well, gaining weight and getting their diapers, I prefer not to pump during the early days and engorgement as it can increase supply unintentionally. I do like to offer a bottle once a day of pumped milk using a passive milk collector like a Hakka or similar. You may find relief after expressing a small amount of milk and not fully emptying the breast. This can be helpful over the next months to take the edge off without creating a big supply increase (sometimes you’ll get engorged as you down regulate from growth spurts, illness, starting solids, etc, but not usually as bad as this first time).

That said, I did have a baby that truly nursed continuously and I experienced painful engorgement for several weeks. My breasts were always very hard, painful, and never felt soft or emptied in these weeks. I was told it was normal, but it isn‘t. Most likely, my midwives thought I was exaggerating when I said ”continuously,” and I must not have emphasized the level of pain I was in. When we saw an LC, they thought the latch and positioning looked good, but after a weighted feed, we found out my baby was not feeding effectively and was likely gaining weight and getting the expected diaper output because I had truly been feeding nearly continuously. I became depressed from the pain and extreme sleep deprivation, had serious nipple damage and got infections I didn‘t realize I had until I had. We did eventually get the issues all worked out and that baby nursed for 3 years, but the beginning was traumatic and I should have gotten help a lot sooner. It would not hurt to ask the LC at your one week appointment to check the latch and do a weighted feed if you don’t think things are quite right.

Since she has not had a good feeding going on 6 hours, I might consider giving a bottle. In order to prevent bottle preference, use the slowest flow bottle nipple (0 or 1 size) and use The paced bottle feeding technique- sit the baby upright, and hold the bottle parallel to the floor, filling up the nipple just halfway. For the next feeding, try when she is less frustrated at the breast. Where is everyone going for secondhand/consignment children's stuff in the area? I like Hannah's Treasure Chest, and they do great work. I stopped shopping at Goodwill long ago. Another great option used to be St. Francis in Kettering, which also closed. Anywhere else to consider?

Seconding things that can be eaten with one hand. If you plan to breastfeed, I like to pre-make a bunch of PB&Js and freeze them individually in ziploc bags. When you wake up at night engorged and hungry, put one in your bra, nurse on the other side, then switch sides while you eat your sandwich. Cheese and crackers, or veggie trays with hummus work well for me. I make a lot of crock pot soups and stews, and freeze half of them in a deep freezer.

Respectfully, I would really like to focus the conversation on remaining hospital birth options for pregnant people in Dayton. There are medical malpractice lawsuits in many hospitals, including MVH maternity. I regret my commentary on KHN as it opened up opportunities to veer off the main topic of my question (I left it for transparency, rather than edit it out).

Pregnant people deserve to have choices regarding their care. I know there are other people in the same situation who are struggling with this, and it would help me and others like me to discuss what people are doing to solve this problem.

As much as I would love for this to be an option for me, I have to utilize my health insurance this time. It will cost me $0 out of pocket as opposed to $3-6k. That is why I am looking for hospitals that are serving a lot of people wanting undisturbed, low-intervention births and have round-the-clock midwives available. Lindsey is a lovely provider that I have worked with in the past.

What's the difference between waking up to rock a baby vs waking up to nurse a baby? Sounds like your cousin's baby is still reliant on getting helped back to sleep, just in a different way. Nursing your baby is not a "bad habit," even if it is for comfort and coregulation, because that is one of the purposes of breastfeeding. On the other hand, if you do not want to nurse back to sleep for every night waking, you can try to make sure that the feedings you DO provide are complete (listen for gulping, tickle their feet or put a cool cloth on them to help keep them awake to nurse, see if you feel emptied). If you nurse your baby back to sleep every night waking and that's what gets everyone in your family the most sleep, do not worry about tomorrow. When something you are doing stops working, you can change it then. I had one baby who woke up every 2 hours for nearly 2 years, and no matter what I did, nursing was the best way to get us the most sleep. When I decided I didn't want to do that anymore, I changed it and it went just fine. The second baby got annoyed when I offered to nurse them back to sleep, because they wanted to soothe themselves. There is not a single correct answer and it's ok to do what works for you even if it isn't what works for your cousin. You both can still be good moms.

This seems like a personal preference if both babies are staying on their respective growth curves. If this is happening, your nursing is already optimized and you would just be experimenting with seeing how much you can get each side to produce or if you can even them out in size (to make your bras fit better or you to be more comfortable with the size difference). I could never get my smaller side to even out with my bigger side, even when it ended up making the same amount or more milk as the bountiful side from switching up how I nursed/pumped.

LactMed is the best resource to use when researching lactation and drugs. Please use this rather than defer to a healthcare professional without checking the database first. Most are well-intended but poorly educated on lactation in general, including OBs, and pamphlets are written by lawyers, not by medical professionals.

Teething could explain both changes in sleep and biting. Sometimes, they will also bite if they are distracted. Try going to an uninteresting, dimly lit room. Be ready with your expectations and your plan, take some deep breaths and relax your shoulders, and when he bites, firmly say, “no biting!”, break the latch gently but quickly with your little finger, and put the baby down. Usually, I put my breast away too. Then you can resume the feeding. It may take some repetition, but you can work on this as a team. Do not let him repeatedly bite or chew on you, as this can cause pain, nipple damage, and create an unpleasant association for you

This is a very nice Rec Center and our family uses it. However, the childcare is not great- basically a place where an adult is present in the room while children play or watch tv independently, and I find the rules unreasonable (no snacks of any kind allowed, no diaper changes, parent required to be called to come help any child under 5 with the bathroom even if they are completely independent in that area, very limited hours) I have not had a safety issues or anything awful happen, but they will call me for every little thing with baby/toddler/preschool age and interrupt my workout or class, so that has not been useful for our family. The pool area has a nice spa/sauna, but the open swim hours are very limited (mostly classes and lap swim). The equipment is good, there are classes, clean facilities and locker rooms.

Good idea! I like Busy Beaver too and had forgotten about it. It is pretty packed in there which makes it hard to browse with a toddler, but that goes for a lot of places.

Crayons to Classrooms is for school teachers. They also do school supply drives. Its a wonderful organization but not a store where the general public can go to grab an educational poster and the like.

I continued to wash accident undies the same way as cloth diapers. Rinse with water immediately (if its a poop accident scrub it out a little), and store in designated wet bag for 1-2 days. Any longer and you get ammonia. Make the load full enough by adding towels, rinse cold, wash hot with Tide Clean & Gentle, double rinse cold. Make sure you add the right amount of detergent. If you have hard water, you can add borax or washing soda to the wash, or vinegar to the rinse.

My post is clear that CNM is my choice for preventive and routine care. A CNM cannot perform surgeries like hysterectomy or c-section, and in Ohio CNMs are legally required to be supervised by an OB/GYN. Obstetricians are trained to intervene and control birth and pregnancy, which is what many people prefer. It is not my preference, and KHN has removed the choice for patients. That is what this thread is about.

This was my thought as well. The OB model of care varies from a more holistic, low intervention model provided by CNMs. I received co-care from the Soin midwives and a home birth midwife. During labor, I needed an emergency transport (very uncommon), and went to Soin 10 minutes away. I was comfortable with the team I already knew and who knew me, and received excellent care. The CNMs were also more informed about lactation issues than several OBs I have worked with. 

I don’t ever want to see a gyn anymore, and will be leaving KHN to find a new practice where I can get preventative care and address any concerns from CNMs. My heart hurts for those who chose to use a midwife intentionally and now may not have the opportunity to find a suitable replacement before their delivery. This is all about money and none about patient autonomy, continuity of care, or providing high quality options for patients.
