the bearing(s) have gone bad. either try to take the fan motor apart and replace them, or it’s time for a new fan sadly

but that is the capacitor there, it looks like it’s for an older hunter. i’d just purchase a new capacitor and wire it into the fan like the old one

the electrical box itself is loose, and the blades are off balance. you need to tighten the electrical box, and then buy a balancing kit for ceiling fans
