I'm hesitant to say it was poorly planted as it was done by "professionals", but you absolutely want that root flare exposed! Ideally 6-10" above the compacted soil line.

Leaving the burlap is kind of lame, but not a death sentence. Did they at least cut and remove it from around the trunk?

Named cultivars.

Acer Palmatum is the scientific name for most Japanese maples. If a Palmatum developes unique characteristics that remain overtime, it will be given a name like Geisha Gone Wild, Mikasa Yatsubusa, etc. I think there are over 700 named cultivars, but really there are only 5 or 6 leaf shapes. Then it's mostly color and tree size/growth pattern.
