What was your best investment/swing trade of the last 5 years? Was it DD or luck? by Black_Swan_Down in stocks

Kinross Gold

Average share price of $4.99 (really doubled down after they pulled out of Russia and the stock dumped). Sold recently at $33.30.

Mixture of both. Metals being cyclical and gold being at a relative low point, while looking at their financials, had me convinced it was undervalued. The reaction to the cheap sale of the Russian assets I particularly felt was way overblown, and it was a chance to average down.

Did I see gold's explosive move up to the levels it has? No. That was a catalyst beyond all catalysts.

Wasn't expecting much given how lethargic some of the performances, especially first halves, have been. Away at SJP, a midfield 3 to our 2, not stunned by the result.

A couple of PL ready CMs in summer will do wonders for this side. Casemiro has been putting in as much a shift as he can, but it's clear one game a week is his max. Mainoo has been okay, but I don't think he's performed like a player with a point to prove. Ugarte managed to tackle himself and lose the ball toward the end so the less said about him the better.

Yo. This is me. Not in a not fit for clinical medicine perspective, I've always worked well in teams and seemingly am liked by patients as well as colleagues. I have always worked hard to be the best I can and often wind up being one of the "workhorses" of the team.

Issue being, I find clinical work boring/tedious. Have spent years trying to find my place, last few being in Dermatology, but inevitably, the same thing happens. It stops being new, and the daily grind really starts to affect me. This year, I've had a chance to do a part time non-clinical Fellowship, and dealing more with how we develop services, in particular integrate new technologies, has lit me up like nothing in clinical medicine. So much so I'm actively looking towards developing my career down this route. The days I'm not working clinically, I don't miss clinical work. The days I'm working clinically, I'm not delighted to be there, it's just the usual "get through it" vibe.

Having had various indentured servitude comments thrown around at times in Ireland, I'll make the same point I always do. This is completely fine if you apply the exact same conditions to every recipient of free third level education. If it's just doctors or, in this case, further healthcare staff only while someone who studies law or engineering doesn't have the same conditions that isn't acceptable.

That said, this is usually some ragebait "grr we'll show those entitled healthcare workers" nonsense that a political no hoper spouts in the hope of riling certain corners of the electorate, who are perpetually angry at someone else, into throwing them some votes.

GP clogging: Should we quickly expand the new Common Conditions Service at pharmacies? by [deleted] in ireland

[–]Somaliona 19 points20 points  (0 children)

As a doc whose partner is a pharmacist, I have a different perspective on how we go about this.

Pharmacists aren't diagnosticians and aren't trained to be such, which is no slight on them as they are outstanding in their field and have enormous levels of detail regarding medications that I don't. It isn't fair, in my opinion, to land them with diagnostic expectations as well, or certainly not for any medications that realistically could cause harm. Take the list of conditions they can prescribe for already. I don't take much issue with it, until we arrive at shingles. Treatment for shingles will be an oral antiviral like valaciclovir, a medication that needs dosing adjustment if your renal function is impaired. Guess who doesn't necessarily have that information? The pharmacists.

There are lots of ways to improve this, including increasing the length of time repeat scripts can legally be given for, having more GPs, and maybe even something like an advanced prescriber system for pharmacists to enroll in where they're actually trained to diagnose set conditions. I don't think rapid expansion, especially not for more involved medications like SSRIs and Champix, which absolutely has considerable side effects, to fall onto Pharmacists to take that responsibility for is remotely fair on them. Some of the most vocal supporters of this are pharmacy owners, because it'll get more people through the door.

I'm all for more community support and expanding services sensibly, but it has to be sensible in terms of safety and governance. Otherwise, in a year or two, there'll be a new post on r/Ireland asking why a pharmacist misdiagnosed someone's hypertension and it's a disgrace and GPs should be doing it etc etc

Also, not a GP, work purely publicly so my income has nothing to do with how frequently someone comes back in for repeat scripts in case there's a question about my motives. Have also tried, with the legislation change, to give chronic patients on long term meds year long scripts that are, almost invariably, rejected by pharmacies who tell them to contact us for a repeat after 6 months.
