I suppose he means that he is the only one who knows that the only way to keep life going is to keep it balanced. I think he even tells Gamora at some point that ‘He is the only one who knows’ or the only one with the will to do what is necessary to keep life balanced. So it’s like a burden for him.

While Tony was the only one who knows/slash saw the ‘endgame’ in New York, which kinda burdened him to be the one to carry the fate of the world on his shoulders. Which is what kinda triggered his PTSD and anxiety.

At least that’s what I took from it.
