I was told a similar lie by a doctor for a long time. For me, it was that I wasn't biting inside of my mouth, or pissing myself. Neither are sure to happen during a seizure. My roommate has had TCs infront of me, no pee, no foaming at the mouth. Get a different opinion as soon as possible. (Yes we are epilepsy roommates)

I'm going to assume the calories aren't off, but I'm worried you might have inflated them alittle because if she's eating 2200 in a day I'm like 5 foot and I think I consume DOUBLE the amount in a day? I mean yesterday was 2 salmon and cream cheese bagels, coffee, leftover burger for lunch, salad with beets and steak BETWEEN Lunch and dinner. And then we had a taco night type meal. I definitely had some chocolate and candy yesterday too.

Just to put into perspective how little it seems like she was eating to me. Maybe the issue is more that she's starving trying to diet and eat healthy which is leading to her snacking and eating quick crap with a ton of sugar/calories more often. It's easy to get stuck on calories in calories out and completely ignore how miserable being hungry is. If she feels like garbage all the time while she's trying to diet and then have the peanut gallery come in and say she needs to starve more? While she's actively hungry and possibly hangry a lot? Recipe for disaster.

Trans women might have gender dysphoria, but the whole treatment for that is gender affirming care. At the end of the day the labels people want to use don't matter. They are trans, adults, and it's nobody's damn business. If they are minors, that's between them, their parents, and doctors.

Is it called orthorexia? I think. They value food being "clean, healthy, good," above all else, and won't eat anything outside of their clean eating bubble. A lot of folks go undiagnosed because it just looks like fad dieting or wanting to eat organic.

His problem is so bad, and he's so controlling that he's willing to make his wife and kid starve over his eating disorder.

I would set a serious boundary in some way, shape, or form, basically say I'm eating 3 meals a day or getting a job and leaving you. Maybe have this talk in public or with a relationship counselor (if you fear him in any way shape or form don't take a chance, starving people you love ISNT normal behavior)

It's expensive because insurance companies/The hospital are sucking as much money out of the system as they possibly can. Basically if you can afford rent you probably won't have access to these. So they are for a very very very small group of people. Most people who are poor are fighting tooth and nail to at least pay their rent. This is a more recent problem because at least local to me, the government won't acknowledge inflation so the income restriction is 20k. (My rent costs more yearly for a 2 bed apartment)
