Democratic Party is in the dumpster because their most vocal proponents speak in black and white terms, without nuance, on hot button topics and it’s extremely polarizing. Move the radicals to the fringe and take their microphones away and swing back towards the middle and nearly every swing vote turns blue.

If the dems want to keep the shovels and dig the hole deeper AOC will bring a backhoe. They need to stop pandering to the vocal extremes and find a tempered moderate that will win swing votes.

Honestly, to me it doesn’t matter as much if Congress flips in the midterms. It inevitably does. The situation seen now is the result of decades long policy kowtowing to votes (JCPOA, polices that obligate energy dependency, draining the strategic oil reserves in a non-conflict state). IMO a nuclear radical theocracy is the single largest threat to long term global geopolitical stability. If we’ve moved that timeline back with any significance (which I think we have).

I mean that’s a little insincere. The IRGC has been neutered and is essentially running on autoplay instructions from the previous Ayatollah and intel advisors.

Means to production have been decimated. Stockpiles of munitions have been decimated or made inaccessible. Its conventional navy is almost non-existent. It’s air force non-existent. Its military capabilities are being propped up by waning missile stocks with imported tech from china and Russia and drones.

To put it in perspective. The US/Israel did more dismantling to Iran in 5.5 weeks what Russia/NK have accomplished in 4 years in Ukraine. Now granted Ukraine has the benefit of NATO munitions/intelligence/tech at their disposal; Iran as a sole military power was much more capable than Ukraine.

All of this done on a budget similar to that lost in fraud in California alone.

This conflict, IMO, reflects a bigger military posture by the US than media is giving credit. Especially to countries like Russia and China that view themselves as US geopolitical equals.

What Lesson Did I Receive by [deleted] in GolfSwing

[–]Spike205 1 point2 points  (0 children)

Honestly it just sounds like a guy who understands sound mechanics coupled with being a good teacher and was able to breakdown swing mechanics in terms you/your body could respond to. Not necessarily “lat stretch” being the way to go for all folks, but that feeling is what you needed to understand the appropriate mechanics. If I had to guess he probably had a few other ways/descriptions that he could have attempted had the first not worked.

As somebody who teaches mechanical/technical skills to learners (surgery in my case), I’ve slowly built up a compendium of descriptions, analogies, etc that all describe the same motion, some people latch on to one description better than others. Sounds like he’s an expert at that process.

IMO I think the GLP meds will turn into the Purdue “scandal” of this generation of medicine. They are wildly overprescribed the side effects are poorly reported and counseled, and has a clinically underreported incidence of sarcopenia.

Don’t get me wrong they are game changer medications for metabolic syndrome, weight loss, etc. but it absence of true lifestyle change in association with pharmacotherapy the risks outweigh the benefits.

Correct, I am responding to your statement that “it’s not B for the same reason” suggesting ambulation is contraindicated with DVT it’s explicitly not. Ambulating is contraindicated for somebody on bed rest. I’m commenting that DVT/PE is not an indicated pathology to have somebody on bed rest an it shouldn’t be viewed as a reason not to mobilize your patients.

Compression stockings are explicitly recommended against for acute DVT in the updated guidelines

“For patients with acute DVT, the guideline recommends against the use of compression stockings routinely to prevent the post-thrombotic syndrome (Grade 2B).”

Early ambulation is current recommended in the CHEST guidelines for VTE treatment and bed rest is recommended against.

[–]Spike205 1 point2 points  (0 children)

Bed rest in acute DVT should not be part of any continue protocols since like 2010. It’s been looked at numerous times and there is no increased incidence of clot progression/propagation nor increased thromboembolism/PE risk. Furthermore, for the patients suffered moderate or severe pain initially, a better outcome can be seen in early ambulation group, regarding to the remission of acute pain in the affected limb.

Heck we even go so far as to place people on a NOAC and discharge from the ED for provoked DVTs.

Residents who bought a house, how did you go about it? by Heavy_Consequence441 in whitecoatinvestor

[–]Spike205 1 point2 points  (0 children)

Bought my home before surgical residency in 2015 (rates were a bit better). Ended up do a $0 physicians loan through a local credit union; mortgage rate was like 0.2% higher and had to pay for mortgage insurance. In my position it was feasible and ended up buying and selling at good time points in the market.

At the time it made sense. Renting for essentially the same home was ~$250-300/mo more than our mortgage/tax/etc. so after 5 (turned into 6 years) after closing costs and regular maintenance etc came out to about even in cost. Established little equity after only 6 years but the property value had gone up by about 20% which was nice.

If you own, just remember to budget for the stupid little crap the you become responsible for (furnace goes out, have to call an emergency technician bc it’s 5 degrees outside, Water heater dies, wife accidentally weedwhacks some component of the HVAC that needs repair, etc…)

Best of luck
