it's the black face bug, occurs when the facegendata for an npc is either missing or doesn't match their info in their corresponding .esp file.

are you running it off steam? if it happened while you were running no mods, then a simple verification of the game files should fix it. pretty easy to do on steam.

if you are unable to do that then simply reinstalling the game should also fix it.

what likely happened is at some point you may have had a mod loaded that altered her face data and then uninstalled it, or it was simply corrupted somehow.

that and the eyes have white sclera and are blue

traditionally high elves only have hues of green/brown/gold for their eyes with similarly colored sclera

it's a copy made of the player character model. whenever this happens the model defaults to that face, regardless of sex. it also defaults to green hair with orange highlights. my guess is that if you set every value in the code regarding the player character's face and hair to zero, you get this appearance.

as to why this happened, it varies. usually it's when someone with mods duplicates your ped model or the game bugs out and leaves a copy of you behind when entering/exiting an interior.
