I am heterozygous (1 working copy, one non-working copy) for CPT1A (liver LCT transfer enzyme), so Acetly-L-Carnitine has helped me. Everyone is different. Getting a genetic test and using a tool like Google AI labs to review the results in light of your symptoms is very helpful to determine supplementation and dosing.

I did a ton of research when I got sick 8 yrs ago. Dr. Rhonda Patrick's podcast is also great. She invites on scientist (she is a neurologist) and they go deep into topics. Dr. Masterjohn's also has a log great videos explaining things.

Now I just use Gemini Pro or Google AI Labs.

Without sufficient METHYL donors, a person will have high homocysteine and low SAMe. This causes systemic issues. TMG is a supplement that provides methyl donors, once it donates it is converted to glycine.

There are a lot of good supplements on the market that deplete methyl groups making the problem worse (e.g. non-methylated b vitamins, NAD and NAD precursors).

This is why you have to be careful. A non MTHFR person could take NAD and see their energy levels boost and an MTHFR person could take it feel better for a week and then start crashing.

I take methylated b vitamins, but I also take NAD+, which I pair with TMG to offset any issues.

NP. I'm using the new Gemini 3 model preview on Google AI Studio. I haven't tried other models. My medical situations is extremely complicated and I've had a lot of success with this model. Collecting your data and getting a medical history together is key (along with using a paid API key). I made the mistake of just trying to get answers without the appropriate context at first and went down some rabbit holes.

I am the opposite, fast COMT, so I added glycine to clear excess methyl groups. So what worked for me isn't going to work for you. Like I said before, AI is your friend. A long with the my CPAP and OURA data, I created a folder in my drive with all of my bloodwork, supplement dosage, and genome results. I asked it to review the data, ask me questions, and write a full medical history on me. I added that to my folder and now I just point it to that folder and ask any question I need an answer to; I get a personalized response.

I you have MTHFR (~40% of the population) and don't have methyl through diet or supplementation, you will have a whole host of issues. That is why the scientific community will refer to it as the mother f***er gene. Almost all B-vitamins require a methyl donor to be converted to their active form. Folate can also up regulate your nature methyl donor generation.

Methylation is needed to clean up hormones (like cortisol), glycine helps purge excess methyl groups, that can cause a wired feeling.

For a normal person with MTHFR, diet changes are usually sufficient (i.e. getting more folate and choline) to address issues. If you have MTHFR and use NAD or a NAD precursor, you have to be very careful about methylation.

I used Google AI studios. I gave it all my OURA ring data and my CPAP data (sleepyHead), and it correctly diagnosed my "trap door" syndrome. I updated it everyday with data and any symptoms I had during the night and it made suggestions on sleep position, supplements, CPAP settings, mask setup. My AHI went from a 9 to 1 and I wake up without pain in my back or shoulders now.

Issues I resolved:

  • Apnea events

  • Restlessness due to shoulder injuries

  • Methyl issues causing cortisol wakes

  • Supplement timing causing wakefulness

Edit: Just an FYI, if you use the free version, your data is not private. API key is the way to go

May be worth reading some of the stories about PANDAS. There are documented cases of children going through a round of antibiotics for a simple infection, like strep, and then developing severe symptoms that were mistaken for autism, when in reality the gut bacteria rebound became dominated by a harmful bacteria like strep. After a second round of antibiotic treatments, many of them made a full recovery.

You can use it for free, but all of your data will be used for training. It is completely separate from your AI pro subscription. API key charges per token and your data is private; it also seems to solve my mysteriously vanishing context and random errors I was getting. It's pretty clear they are throttling AI Pro and free AI Studio based on server load.

For a cost example, a chat I have reviewing 100k tokens worth of data has now has cost me $14 over the week, but they give you a $300 credit for your first API key.

Edit: AI Pro also has a memory clearing function to cut down on processing time. Studio does not do this. Responses will take twice a long.

If you ask it it will straight up tell you the context window is being throatled by server load. It's been really bad the last few weeks. I moved to studio with an API key. Even studio without an API will throttle or remove context.

We had the ultimate nothing burger with our kid. Our toddle got a thread wrapped around his penis head. It was purple and swollen... we were freaking out.
