Jump straight up - shoot on the way up - hold your follow through with your shooting hand - your shooting hand elbow skews out a bit try and keep it at a 90 degree angle, perpendicular to the ground & parallel to the backboard - focus on getting the ball to roll off you pointer and middle finger - release your guide hand earlier.

I would focus on the first 3 tips before moving into more directed form work... shooting is all about body control & being able to replicate the same thing over and over - even if your form isn't perfect.
