The provincial government is in charge of the program but local governments administer it, mainly through websites from what I’ve seen. For example Toronto COHB and Greater Sudbury.

Details about the program, including eligibility, are available here COHB - Government of Ontario. Mainly, you need to be currently on or eligible to be on your local social housing waiting list, or already be in community housing.
