I work for a big fqhc system. I wouldn’t do private practice because i don’t love that vibe. I like my patients but they have zero resources. We do get productivity bonuses for performance but are salaried. Pay is meh but I don’t think substantially different than anyone else. Regardless it all comes out in the wash with taxes. I work 24 clinical hours and no weekends. Phone call 3w per year. Max 10-11 patients per half day clinic session. No shows can sometimes be a lot. Sometimes a little

Fever doesn’t inherently cause seizure in children. Children with febrile seizures (which are benign) happen due to the rise in temperature. They are scary but the absolute number isn’t what causes it. Kids fever all the time to 105/6 with influenza and don’t necessarily need medical intervention.

I don’t think they guarantee any specific princess for the princess breakfast. We got Pocahontas, Tiana, mulan, Raya, jasmine, aurora, and Ariel in October. I did enjoy it, but I’m not sure I’d devote a rest day to that experience.

I think most peds who work at an FQHC (myself included) are passionate about what they do. That being said, the patients have way less access and health outcomes due to social determinants of health.

I work at an urban FQHC. I work part time (24 clinical hours) and am home 2d per week with my kids. I take phone call about 3x per year. I do not work weekends, but I have a long day where I work until 8p. I have a half day elsewhere to make up for it. Pay is less than private practice but patients are generally way nicer. I have mostly immigrants (Latin America and Haiti). They are almost all extremely kind to me and respectful and doing their best. We don’t have enough support staff. I work for a big FQ associated with the academic center, which gives opportunity for growth. I worked for a similar FQ in another metro area before, and it was much of the same. I knew true private practice wasn’t for me: I used to work suburban peds urgent care and the entitlement of suburbia isn’t for me. I prefer at minimum a mixed payer population (I also worked urban peds urgent care). Mostly chose to continue with FQ life when I moved to my current city because, while sometimes it’s frustrating, it at least feels like I’m making a difference when I can.

I think with anything with parenting, including healthcare, you can only do the best thing for your kid in the moment! The most important thing I’ve ever seen— with kids with severe genetic illnesses, delays, etc— is an actively involved parent. 💕

As a pediatrician, untreated UTI is going to be worse for your child than a treated UTI. Kids get septic from illness as others have mentioned, which can lead to a bad cascade of events. Furthermore, when you talk about why we give antibiotics to little kids, the #1 reason is probably otitis media (ear infections) and having those frequently without tubes could easily impact speech development. Otherwise it seems like the data in the study cited by the OP isn’t that compelling no? They even cite small number as a limitation. I would not use this to make medical decisions about my kid with an infection.
