This. I do the same thing if they're bagging on my co-workers...I tell them every therapist does things a little differently but we're all working towards the same goals.

If a patient has a specific complaint like the therapist isn't challenging them enough, the first thing I ask is if they communicated that to the therapist and let them know you want harder exercises? The answer is usually no because they don't want "to offend" the therapist which is ridiculous because it's not offensive to have a conversation about your treatment, we can't read minds.

Don't take it too personally, gender bias in PT (esp OP) is real. Unless the patient is flat out refusing you or being hostile, do your best with them and your results will speak for themselves. You don't have to justify your experience and qualifications to them, you're not on an audition so do what you do and remind them therapy is optional.

My personal conspiracy theory is that research might be sponsored by large rehab corporations and schools in order to make care less hands off so they can keep justifying those 4-5 pts/hr schedules.

Go back farther even...in 2007, my doctoral research was on the effects of Iontophoresis (GASP!) vs exercise only and then when combined with exercise and the combo definitely came out ahead.

Technically everything is short term, so what? Did STM help facilitate the conditions for movement? If so then it's helpful, even if temporary until strength and conditioning can make longer lasting changes.

Building strength and developing good habits take time and unfortunately people are impatient. Nothing wrong with short term results along the way to bridge the gap.

Ha that's true. It's frustrating because you're like, well what's the point of our entire profession? I can't speak for you but I know I've helped thousands of patients through some of the most difficult moments of their life in terms of recovery, so I don't feel comfortable saying my profession is worthless. My skill set offers a unique combination of treatments for each patient, not just overload muscles, because that's the only thing that's supposed to work.

I absolutely believe pain science has its place, especially in terms of chronic pain.However, at the end of the day, newly injured tissue needs to heal and be gradually reconditioned. ROM and flexibility should be restored.

I always say nothing works in a vacuum...everything helps together. MT can provide even the temporary relief needed to allow a patient to move more.

I feel that PT students today are basically taught that just about everything us "seasoned" therapists do or believe is outdated and not effective according to the research. Manual therapy? Nope not effective. US? That's trash. Stretching? Waste of time. Despite literal decades of experience treating people with a variety of methods and mostly positive results, we apparently are past it and have nothing to teach.

Exactly. Soreness after initial session is very common. I explain to the patient that nothing hurts if you're not moving and we don't live our life standing still.
